摘 要:目的:环境内分泌干扰物对复发性流产影响的系统评价。方法:计算机检索Pubmed、Web of Science、Google学术、中国知网、维普、万方数据库,并辅以手工检索,检索时限为2000~2015年,纳入不明原因复发性流产患者及有职业暴露的患者,分析环境内分泌干扰物对复发性流产的影响。采用Revman5.3软件对纳入文献数据进行Meta分析,计算危险因素的比值比(OR)及95%可信区间(CI)。结果:共筛选出相关文献10篇,其中中文文献5篇,英文文献5篇。Meta分析结果显示;环境内分泌干扰物在不明原因复发性流产与对照组暴露水平差异具有统计学意义(Z=7.50,OR=0.54,95%C10.40~0.68,P〈0.00001),环境内分泌干扰物职业暴露是自然流产的危险因素(Z=5.04,OR=1.77,95%CI1.42—2.21,P〈0.00001)。结论:环境内分泌干扰物可能是复发性流产重要病因,环境内分泌干扰物职业暴露导致自然流产增加,建议生育期人群在备孕期、孕期避免环境内分泌干扰物接触。Objective: A systematic review and Meta-analysis in effects of estrogens disruptor chemicals (EDCs) to unexplained recurrent spontaneous abortion. Methods: We searched the database between January 2000 to December 2015 ,including PubMed,web of science,CNKI and so on and supplemented by hand searching. We selected the patients of recurrent spontaneous abortion(RSA) and the patient with occupation exposure, analyzed the impact of estrogen disruptors chemicals on pregnancy. Data were analyzed by revman 5.3 software,fixed effect Meta analysis model was adopted to calculate the combined odds and 95% confidence interval. Results:10 literatures,including 5 Chinese literatures and 5 English literatures,were extracted. The difference of Environmental estrogen disruptor chemicals exposure level in recurrent spontaneous abortion patients and control group is statistically significant. The occupational exposure to the environment estrogen is a risk factor for spontaneous abortion. Conclusions:Environmental estrogen disruptor chemicals are likely to be an important cause of unexplained recurrent spontaneous abortion. Environmental estrogen disruptor chemicals professional exposure will lead to an increased morbidity of spontaneous abortion. So the child-bearing period woman, bothin pregnancy and during pregnancy, should avoid environmental estrogen exposure.
