摘 要:目的:观察头针"额旁1线"对再灌注性心律失常大鼠延髓头端腹外侧区神经型一氧化氮合酶(n NOS)表达的影响,探讨头针防治再灌注性心律失常的中枢作用机制。方法:Wistar大鼠随机分成假手术组(S组)、模型组(IR组)、电针治疗组(EA组),每组8只。采用结扎左冠状动脉前降支30 min,再灌注15 min制备大鼠心肌缺血再灌注模型,以延髓头端腹外侧区n NOS表达水平、左交感神经放电活动及再灌注性心律失常发生率为观察指标。结果:IR组大鼠延髓头端腹外侧区n NOS降低,左交感神经放电活动明显增强,再灌注性心律失常发生率升高,与S组比较差异有统计学意义(P<0.01);EA组大鼠与IR组比较,延髓头端腹外侧区n NOS表达显著上调,左交感神经放电活动明显减弱,再灌注性心律失常发生率降低(P<0.05)。结论:电针可通过调整再灌注性心律失常大鼠延髓头端腹外侧区n NOS表达水平,从而降低左交感神经放电活动,发挥其抗再灌注心律失常作用。Objective:To explore the mechanism of electroacupucture(EA) of scalp-points in the management of reperfusion arrhythmias by examining neuronal nitric oxide synthase(n NOS) expression in rats rostral ventrolateral medulla(RVLM).Methods:Wistar rats were randomly divided into the sham(S),model(IR),EA groups(n = 8).Myocardial ischemia-reperfusion model was induced by ligation of the left anterior descending coronary artery for 30 min,followed by 15 min reperfusion.All the indexes(the incidence of reperfusion arrhythmia,left sympathetic nerve activity andnN OS expression in RVLM) were measured.Results:Compared with the S group,the incidence of reperfusion arrhythmia and left sympathetic nerve activity were significantly increased and the nN OS expression was severely reduced in the IR group(P〈0.01),whereas in comparison with the IR group,the incidence of reperfusion arrhythmia and left sympathetic nerve activity were markedly lowered(P〈0.05).The nN OS expression was significantly up-regulated in the EA groups.Conclusion:EA can exert its anti-reperfusion arrhythmias by adjusting the nN OS expression of RVLM,thereby reducing the left sympathetic nerve activity.
