摘 要:目的探讨慢性阻塞性肺疾病(慢阻肺)患者血浆降钙素(PCT)水平变化以及与BODE指数的相关性研究。方法选择2014年5月至9月收治的AECOPD患者120例,按GOLD综合评估按病情由轻至重分组,A、B、C、D四组,以上患者经治疗后病情基本恢复到急性加重前的状态为稳定期组,测定肺功能、六分钟步行距离,评估呼吸困难程度,计算体质指数,计算BODE指数,选取同期于我院体检的健康人80例为健康对照组,分别在慢阻肺急性加重期、稳定期以及健康组采空腹静脉血,用酶联免疫吸附测定法(ELISA)测定血浆PCT水平,分析慢阻肺患者血浆PCT水平变化以及在不同BODE指数级别中的变化。结果慢阻肺急性加重期患者血浆PCT显著高于慢阻肺缓解期、健康组,并随病情评估分级升高,组间差异均有统计学意义(P<0.05)。按BODE指数级别分级,PCT水平逐级增高,组间差异具有统计学意义(P<0.05),BODE指数1级组的PCT水平与健康组间差异无统计学意义(P>0.05)。相关分析显示,PCT水平与BODE指数呈正相关(P<0.05)。结论慢阻肺患者血浆PCT水平明显升高并与BODE指数有良好的相关性,,临床可将其用于作为评估患者病情程度及预后的指标。Objective To explore the correlation between PCT level and BODE indexes in patients with chronic obstructive pulmonary disease. Methods A total of 120 patients with AECOPD were selected from May to September 2014, and they were divided into the A, B, C and D groups according to GOLD comprehensive evaluation by the disease from mild to severe. When the symptoms like cough, sputum and shortness of breath recovered to sta- bility from acute exacerbation, their lung function, 6-minute walking distance, dyspnea, constitutional index and BODE index were recorded and analyzed. At the same time, 80 healthy people who went through physical examination in our hospital were taken as the control group. Their empty stomach venous blood was collected from AECOPD, COPD patients at stable stage and the control group, and their plasma PCT level was detected by ELISA method to analyze the changes of plasma PCT levels in patients with COPD as well as changes in the different level of BODE in- dex. Results The plasma level of PCT in patients with AECOPD was significantly higher than that in COPD patients at remission phase and the control group, which increased with the assessment of the severity of the disease ( P 〈 0. 05 ). Classified by the BODE index level, PCT level increased grade by grade (P 〈 0.05 ), but there was no significant difference in PCT level between the first BODE group and the control group ( P 〉 0. 05 ). Correlation analysis showed that the level of PCT was positively correlated with the BODE index ( P 〈 0.05 ). Conclusion The plasma level of PCT is significantly increased in COPD patients and has a good correlation with BODE index, which can be used as an index to assess the degree of patient' s condition and prognosis.
