摘　　要：目的观察在社区获得性肺炎(CAP)患者中血清脂联素变化水平,并探讨与肺炎严重程度(SCAP score)评分、降钙素原(PCT)、C-反应蛋白(CRP)的关系。方法采集134例CAP患者入院时的数据,依据最新的IDSA/ATS的诊断标准区分重症及轻中度CAP,分别依据SCAP score评分系统予以评分,并检测入院当日(D1)、第七天(D7)血清脂联素水平,同时检测血清PCT、CRP含量。并与32个健康对照组的血清脂联素数据予以比较。结果 CAP患者血清脂联素水平在D1和D7有明显的区别(7.36±2.97 ug/m L和9.44±2.65 ug/m L,P=0.04),入院D1与对照组比较(11.2±2.23 ug/m L)明显降低(P<0.01)。D1脂联素水平和PCT及SCAP score比较有明显的负相关关系(分别为r=-0.55,P<0.01和r=-0.59,P<0.01)。结论在这个初步的临床实验中,CAP患者D1脂联素水平和正常对照组是明显降低的,脂联素分别和PCT及SCAP score评分呈明显的负相关关系。这些数据提示脂联素在CAP的炎症反应阶段可能起到重要的作用。Objective To evaluate the relationship between the level of serum adiponectin in community-acquired pneumonia （CAP） patients and severe community-acquired pneumonia （SCAP） score （SCAP score）, proealcitonin （ PCT）, and C-reactive protein （CRP） respectively. Methods 134 CAP patients were enrolled. The severity of community-acquired pneumonia was separated according to IDSA/ATS severity pneumonia criterion and scored by SCAP scoring system. The levels of adiponectin and PCT and CRP were detected on Day 1 and 7. The Results were compared with 32 healthy controls. Results The mean plasma adiponectin concentration was significant different on D1 and D7 （7.36 ±2. 97 ug/ml and 9.44 ± 2. 65 ug/ml, P = 0. 04）, and it was significantly lower in the CAP group on D1 than in the control group （ 11.2 ±2. 23 ug/ml） （P 〈0. 01）. There was a negative correlation between adiponectin and PCT and SCAP score on D1 （r = -0. 55, P 〈 0.01; r = -0. 59, P 〈 0. 01 ）. Conclusion In this preliminary study, CAP illness is associated with lower adiponectin as compared with controls, and there is a negative correlation between adiponectin and PCT and SCAP score, which indicates that adiponectin may play an im- portant part in the inflammatory response in patients with CAP.
