摘  要:目的研究由乙肝病毒(HBV)感染导致的慢性乙肝及肝硬化与肝癌患者外周血B细胞表面PD-L1的表达特性,分析其与乙肝患者临床病症的相关性。方法采用多色荧光标记和流式细胞术检测了健康志愿者(Healthy control,HC)、慢性乙肝(Chronic hepatitis B virus infection,CHB)及肝硬化(Liver cirrhosis,LC)与肝细胞癌(Hepatocellular carcinoma,HCC)患者外周血CD19^+B细胞表面PD-L1的表达;采用酶联免疫吸附测定法(ELISA)检测乙型肝炎血清学标志;采用荧光定量PCR法检测HBV DNA,检测结果以>5×102copies/ml判断为阳性;采用罗氏全自动生化分析仪的检测肝功能指标谷丙转氨酶(ALT)和谷草转氨酶(AST)。数据统计及相关性分析采用Graph Pad Prism 6.0软件。结果将乙肝患者病例分为慢性乙肝组(CHB)和乙肝进展组(LC^+HCC)。1)与HC组相比:CD19^+PD-L1^+/CD19^+百分比在两病例组中均呈显著上升;淋巴细胞中CD19^+的百分比在两病例组中均无显著性变化;上述指标在两病例组中均无显著性差异。2)在CHB组中:CD19^+PD-L1^+/CD19^+百分比与肝功能指标AST呈正相关,与HBV病毒载量则无相关性;在LC^+HCC病例组中,CD19^+PD-L1^+/CD19^+百分比与肝功能指标ALT和AST均呈正相关,与HBV病毒载量则无相关性。3)两病例组中淋巴细胞中CD19^+的百分比与HBV病毒载量、肝功能指标ALT、AST间均无显著相关性。结论与HC组相比,乙肝患者外周血CD19^+PD-L1^+细胞亚群的表达发生了明显变化,为乙肝病程的进展提供了有价值的免疫学参考指标。Objective To determine the characteristics of PD-L1 expression on circulating CD19^ + B lymphocytes in Pe ripheral blood of patients with HBV infection ,which includ CHB, LC and HCC ,and further analyze their correlations with clinical parameters. Methods Expression of CD19 ^+ immune cell subsets distribution and PD-L1 in CD19^+ B cell surfacewere determined by Muhicolor fluorescent labeling and flow cytometry in healthy volunteers ( Healthy control, HC), chronic hepatitis (Chronic hepatitis B virus infection, CHB) and cirrhosis (Liver cirrhosis, LC) and hepatocellular carcino- ma (Hepatocellui[ar carcinoma, HCC) patientsrespectively.. HBV serological markers were measured using the Enzyme- linked immunosoIbent assay Kits. HBV viral load was determined with HBV LC PCR Kit. Detection results 〉 5 - 102 cop- ies/ml judged as positive o ALT and AST, which reflect liver function, were analyzed by using Roche Automatic Biochemistry Analysis Meter. Statistics and correlation analysis using GraphPad Prism 6.0 software. Results The patients with HBV infection were calegorized into 2 subgroups: CHB and LC plus HCC. Compared with HC ,the CD19 + PD-L1^+/ CD19^+ percentage significantly up-regulated in Peripheral bloodof two subgroups of HBV infected patients. CD19 ~ percentage of Lym-phocytes in two groups showed no significant change; and Two case group had no significant difference, the frequency of CD19^+ PD-L1^+ subsets was negatively correlated with HBV DNA load, while waspositively correlated with AST levels In CHB subgroup ^+ In LC plus HCC subgroups, the frequency of CD19 ^+ PD-L1 ^+ subsets was positively correlated with ALT and AST levels respectively ,while have no correlation to HBV viral load. The CD19 ^+ percentage in lymphocytes have no significant correlation with HBV viral loadandALT, AST. Conclusion PD-L1 expression on CD19 + B lymphocytes in Peripheral blood of patients with HBV infection changed obviously, Compared with HC group, which could provided valuable
