摘 要:目的在心脏手术中应用血液管理,以有效减少异体血液的输注,节约血液资源。方法本院2013-2014年4 663例年龄>16周岁成人心脏体外循环下开胸手术如二尖瓣置换术,双瓣置换,主动脉瓣置换,房间隔缺损修补,主动脉根部重建、多瓣膜手术、胸主动脉手术等,其中2013年2 276例,2014年2 387例,男性2 398例,女性2 265例,年龄<50周岁的2 492例,大于50周岁的2 171例。结果 2014年相比2013年患者的输血费用减少,有极其显著性统计学差异,女性相比男性患者在输血费用,住院费用和住院时间均低,且有极其显著性差异。<50岁年龄组相比>50岁患者在输血费用,住院费用和住院时间均低,且有极其显著性差异。结论血液管理在心脏手术的应用是有效和可行的,可有效减少成人心脏手术异体血液的输注。Objective To study the clinical application of blood management in thoracic surgery operation. Methods 4 633 patients with thoracic surgery operations, older than 16 years from 2013 to 2014, were divided into 2013 group (n = 2 276) and 2014 group (n=2 387), male group (n =2 398) and female group (n =2 265), younger than 50 years old group and older than 50 years old group. Results In the 2013 and 2014 groups, the length of stay and hospitalization ex- pense were not statistically significant (P 〉 0.05), while blood transfusion cost in 2014 group was lower than in 2013 group (1.77 +4. 01 thousands yuan vs 1.38 +3. ll thousands yuan, P 〈0. 01 ). In the female and male groups, length of stay, hospitalization expense and blood transfusion cost in female group was lower than in male group (P 〈 0. 01 ). In the groups of younger than 50 years and older than 50 years, the length of stay, hospitalization expense and blood transfusion cost in fewer than 50 years group was lower than in older than 50 years group ( P 〈 0. 01 ). Conclusion Blood management can be safe and feasible for thoracic surgery operation. Blood management could be achieved to reduce the use of blood products transfu-sion in patient with thoracic surgery operation.
