母血和极低出生体重早产儿脐血细胞因子IL-6、IL-8、TNF-a、IFN-r水平及相关性研究

摘　　要：目的探讨母血、极低出生体重早产儿脐血中白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、肿瘤坏死因子-a(TNF-a)、干扰素-r(IFN-r)的水平及相关性研究,为治疗提供参考依据。方法收集2013年3月-2014年3月宁波市妇女儿童医院产科胎盘病理活检结果母亲为绒毛膜羊膜炎68例,其对应的极低出生体重早产儿也为68例,再收集正常母亲及正常出生的婴儿20例作对照;采用ELISA方法分别检测母血、脐血Il-6、IL-8、TNF-a、IFN-r含量。结果绒毛膜羊膜炎的母血IL-6、TNF-a、IFN-r含量明显高于正常母血IL-6、TNF-a、IFN-r含量,差异有统计学意义(P<0.05);绒毛膜羊膜炎组母血和脐血IL-6、TNF-a、IFN-r呈正相关(P<0.05)。结论母血细胞因子IL-6、TNF-a、IFN-r含量的测定对早期预测宫内感染,预防极低出生体重早产儿的出生有一定的临床价值。OBJECTIVE To explore cytokine levels and correlations of IL-6,IL-8,TNF-αand IFN-γin maternal serum and umbilical cord serum of very low-birth weight preterm children,so as to provide references for the treatment.METHODS According to the biopsy result of prostate biopsy,68 cases of women with chorioamnionitis from Laboratory Department of Ningbo Women Childrens Hospital were chosen from Mar.2013 to Mar.2014;68cases of the infants which were very low-birth weight preterm children were also chosen;and 20 cases of healthy pregnant women and 20 cases of the infants were chosen as control.Enzyme linked immunosorbent assay（ELISA）was used to detect the levels of IL-6,IL-8,TNF-α,and IFN-γin maternal serum and umbilical cord serum.RESULTS The maternal serum levels of IL-6,TNF-αand IFN-γin women with chorioamnionitis were significantly higher than those in healthy women（P〈0.05）.There was positive correlation of IL-6,TNF-αand IFN-γbetween maternal serum and umbilical cord serum with chorioamnionitis（P〈0.05）.CONCLUSION Measurement of IL-6,TNF-αand IFN-γlevels in maternal serum has a certain clinical value for the early prediction of intrauterine infection and prevention of the birth of very low-birth weight preterm children.
