复方祛白汤联合穴位刺络灸治疗外阴硬化性苔藓疗效及对血清IL-18、IFN-γ的影响

摘　　要：目的观察复方祛白汤联合穴位刺络灸治疗外阴硬化性苔藓疗效及对血清白细胞介素(IL)-18、γ干扰素(IFN-γ)的影响。方法将92例外阴硬化性苔藓患者随机分为2组,对照组46例给予穴位刺络灸治疗,研究组46例给予复方祛白汤联合穴位刺络灸治疗,2组疗程均为2周。记录2组治疗前后中医症状评分,比较2组治疗后临床疗效,检测2组治疗前后IL-18、IFN-γ以及细胞免疫指标水平。结果治疗后,2组中医症状评分较治疗前显著降低(P均<0.05),且研究组显著低于对照组(P<0.05);研究组治疗总有效率显著高于对照组(P<0.05);2组IL-18、IFN-γ水平均较治疗前显著升高(P均<0.05),且研究组均显著高于对照组(P均<0.05);2组CD3^+、CD4^+和CD4^+/CD8^+水平均较治疗前显著升高(P<0.05)、CD8^+水平均较治疗前显著降低(P<0.05),且研究组CD3^+、CD4^+和CD4^+/CD8^+均显著高于对照组(P均<0.05),CD8^+显著低于对照组(P均<0.05)。结论复方祛白汤联合穴位刺络灸治疗外阴硬化性苔藓,可升高血清IL-18、IFN-γ水平,疗效显著。Objective It is to observe the effect of compound Qubaitang combined with acupoint acupuncture moxibustion in the treatment of vulvar lichen sclerosus and its influence on serum interleukin -18 （IL- 18） and interferon gamma （IFN -γ）. Methods Ninety-two patients with vulvar lichen sclerosus were randomly divided into 2 groups, 46 cases in control group were given acupoint acupuncture moxibustion therapy, 46 cases in study group were given compound Qubaitang combined with acupoint acupuncture moxibustion therapy, the course of treatment was 2 weeks in the two groups. TCM symptom scores were recorded before and after treatment in the two groups, the clinical efficacy of the two groups after treatment were compared, the levels of IFN - γ, IL - 18 and the cell immune index were detected in the two groups before and after treatment. Results After treatment, the scores of TCM symptoms in the two groups were significantly lower than before treatment （ all P 〈 0.05 ） , and those of the study group was significantly lower than the control group （P 〈 0.05 ） ; the total effective rate of the study group was significantly higher than that of the control group （P 〈 0.05） ; the levels of IL - 18 and IFN - γ in the two groups were significantly higher than before treatment （ all P 〈 0.05） , and those of the study group were significantly higher than the control group （all P 〈 0. 05） ; the levels of CD3^＋ , CD4^＋ and CD4^＋/CD8^＋ value were significantly higher and the CD8^＋ levels were significantly lower than before treatment （ all P 〈 0.05 ） , and the improvement of the study group were significantly better than the control group （all P 〈 0.05）. Conclusion Compound Qubaitang combined with acupoint acupuncture moxibustion therapy in the treatment of vulvar lichen sclerosus, can increase the levels of serum IL - 18, IFN -γ, and the curative effect is remarkable.
