老年高血压患者血清抵抗素及脂联素水平的相关性研究

摘　　要：目的探讨老年高血压患者血清胰岛素抵抗水平及其与血清脂联素间的相关性。方法以300例青年高血压患者、300例中年高血压患者及300例老年高血压患者作为观察对象,对比3组间血清脂联素及抵抗素含量差异。同时根据高血压分级将老年高血压患者分为Ⅰ级组、Ⅱ级组及Ⅲ级组,对比3组间血清脂联素及抵抗素含量差异,分析老年高血压患者血清脂联素含量、抵抗素含量及血压水平间的相关性。结果不同年龄段的3组高血压患者血清脂联素及抵抗素水平存在显著的差异,老年组血清脂联素含量最低,抵抗素水平含量最高(P<0.05)。老年组不同高血压级别患者血清脂联素及抵抗素水平存在显著的差异,高血压Ⅲ级组血清脂联素含量最低,抵抗素水平含量最高(P<0.05)。老年高血压患者血清脂联素与收缩压(SBP)、舒张压(DBP),血清抵抗素与SBP、DBP,以及血清脂联素与抵抗素含量间均存在显著的相关性(P<0.05)。结论高血压患者随着年龄的增长,血清脂联素含量降低,抵抗素水平含量增高,且老年高血压患者血清胰岛素抵抗及血清脂联素与其血压水平存在显著的相关性,临床可将血清脂联素及抵抗素水平作为治疗老年高血压的临床方向。Objective To investigate the level of insulin resistance in the elderly patients with hypertension,and to investigate the correlation between insulin resistance and serum adiponectin. Methods 300 young patients,300 middle aged patients and 300 elderly patients with hypertension were enrolled in this study. The levels of insulin resistance and serum adiponectin were detected and compared among the three groups. According to the level of blood pressure,the elderly patients with hypertension were divided into grade Ⅰ group,grade Ⅱ group and grade Ⅲ group. The levels of insulin resistance and serum adiponectin were detected and compared among the three groups. In addition,the correlation between the blood pressure,serum adiponectin and resistin levels in elderly hypertensive patients was analyzed. Results There was significant difference in serum adiponectin and resistin level among the three groups with different ages（ P〈0. 05）. The serum adiponectin level was the lowest,and the resistin level was the highest in the elderly patients（ P〈0. 05）. There was significant difference in serum adiponectin and resistin level among the three elderly groups with different levels of blood pressure（ P〈0. 05）. And the serum adiponectin level in grade Ⅲ group was the lowest,the resistin level was the highest（ P〈0. 05）. In elderly patients with hypertension,there was significant correlation between serum adiponectin and SBP,DBP,between serum resistin and SBP,DBP,and between the levels of serum adiponectin and resistin（ P〈0. 05）.Conclusions In the elderly patients,with the increase of age,the level of adiponectin is decreased and the level of resistin was increased. The levels of serum insulin resistance and serum adiponectin are significantly correlated with blood pressure level. The clinical would make serum adiponectin and resistin levels as the direction of treatment of hypertension in the elderly patients.
