摘  要:目的:探讨禾肾丸对阿霉素肾病大鼠肾组织磷酯酰肌醇3激酶(phosphoinositide-3-kinase,PI3K)、人第10号染色体缺失的磷酸酶(PTEN)表达的影响。方法:采用静脉注射6 mg/kg阿霉素以复制大鼠阿霉素肾病模型;大鼠随机分为空白组、模型组、依那普利组和禾肾丸组。予相应药物干预第2周、第4周、第6周及第8周,留取尿液标本,观察尿蛋白定量的动态变化,取肾组织采用免疫组化法及Western Blot法检测大鼠肾脏PI3K及PTEN蛋白的表达情况。结果:模型组24 h尿蛋白定量逐渐增高,均高于同期空白组(P<0.05);禾肾丸组、依那普利组随灌胃时间延长尿蛋白定量逐渐降低,均明显低于同期模型组(P<0.05)。免疫组化及Western Blot结果显示,PI3K主要表达于肾脏肾小球系膜区,肾小管上皮细胞无明显表达,主要表达部位为脏层上皮细胞的胞浆;PTEN则主要表达在肾小球及肾小管的细胞核中,空白组大鼠肾脏中可见强阳性表达,造模组大鼠肾脏表达较空白组少。PI3K在灌胃2周及4周时均逐渐上升,灌胃6周达到高峰,第8周开始下降;PTEN在灌胃2周及4周时逐渐下降,第6周最低,第8周开始升高,其变化趋势与PI3K相反;禾肾丸组和依那普利组大鼠肾组织PI3K及PTEN蛋白表达均明显低于模型组,差异有统计学意义(P<0.05)。结论:禾肾丸降低慢性肾脏病蛋白尿的机制可能与其降低肾组织PI3K及PTEN蛋白表达相关。Objective: To discuss the effects of Heshen Pill on protein expression of phosphoinositide-3-kinase( PI3K)and phosphatase and tensin homologue-deleted chromosome ten gene( PTEN) in kidney of adriamycin-induced nephropathy rats. Methods: Adriamycin-induced nephropathy model was copied by intravenous injection of adriamycin at dose of 6 mg / kg.Then the rats were randomly divided into the blank group,model group,enalapril group and Heshen Pill group. The urine samples were collected on the 2nd,4th,6th,and 8th week after the intervention of corresponding drugs and the dynamic changes of urinary protein quantity were observed,the kidney tissue were collected,the expression of PI3 K and PTEN in rat kidney were detected by immunohistochemistry and Western Blot. Results: The 24 h urine protein quantity in the model group was gradually increased,which higher than that in the blank group at the same timepoint( P〈0. 05); the urine protein quantity in the Heshen Pill group and enalapril group were gradually decreased with the intervention time increasing,which significantly lower than that in the model group at the same timepoint( P〈0. 05). The results of immunohistochemistry and Western Blot showed that PI3 K was mainly expressed in glomerular mesangial region,and there was no obvious expression in renal tubular epithelial cells,cytoplasm of the epithelial cells in visceral layer was the main site; PTEN was mainly expressed in the nucleus of glomerular and tubular cells,strong positive expression were observed in the kidney of rats in the blank group,while the expression in the model group was less than that in the blank group. The expression of PI3 K was gradually increased on the 2nd and 4th week,the peak reached on the 6th week and the expression was decreased from the 8th week. The expression of PTEN was gradually decreased on the 2nd and 4th week,the bottom reached on the 6th week and the expression was increased from the 8th week,which showed opposite change trends with PI3 K.
