摘 要:目的建立紫杉醇涂层支架治疗尿道狭窄的动物模型并进行实验研究。方法选取体重15~20 kg的成年雄性比格犬。通过电凝灼烧法建立尿道狭窄犬模型。6条紫杉醇涂层支架(药物支架)和6条不含药物的裸支架(对照支架)被交替放置在6只雄犬尿道的近端和远端之间的尿道狭窄段。放置支架60 d后,尿道逆行造影观察尿道支架及周围内皮情况,对尿道支架的造影图像进行量化冠脉分析法(QCA)分析。结果造影图像可见模型犬尿道灼伤处出现明显的尿道狭窄。经尿道逆行造影发现支架处尿道狭窄均有不同程度增粗,但QCA分析结果显示药物支架组比裸金属支架组有更好的狭窄扩张作用,其在输尿管支架植入的中点和远膀胱侧的扩张作用均明显优于普通金属支架(1.10±0.26比0.43±0.06,1.07±0.12比0.37±0.15,P〈0.05)。不同剂量的紫杉醇涂层支架其狭窄扩张作用未显示显著差异。结论紫杉醇涂层支架能有效扩张电灼伤形成的尿道狭窄。药物洗脱支架对尿道狭窄可能有潜在的治疗价值,值得进一步的研究。Objective To evaluate the safety and efficacy of paclitaxel eluting metal stent in the treatment of urethral stricture in canine models.Methods The experimental model was the male canine.We establish urethral stricture model by electrocoagulation.6 paclitaxel eluting metal stents and 6 bare metal stents were placed alternately between the proximal and distal artificial damage stricture urethra in 6 male dogs.QCA analysis were taken after 60 days with stent placement.Results Urethral stricture was clearly shown in canine models by electrocoagulation.The efficacy when used paclitaxel-eluting metal stent was better than control stent,especially in the middle and distal of stent(1.10±0.26 vs.0.43±0.06,1.07±0.12 vs.0.37±0.15,P〈0.05).But no difference was showed in different doses of paclitaxel coating stent.Conclusion These preliminary experimental study results suggest that the paclitaxel eluting metal stent is more efficient than the bare metal stent when used within the canine urethral stricture model.
