胎膜早破期待时间对产褥感染的影响及护理对策

摘　　要：目的探讨胎膜早破不同期待时间与产妇产褥感染的影响,提出护理对策促进母婴健康。方法将920例孕晚期胎膜早破孕妇分为:A组(自然破膜时间至胎盘娩出<12h,531例);B组(自然破膜时间至胎盘娩出≥12h,但<24h,307例);C组(自然破膜时间至胎盘娩出时间≥24h,82例)。比较三组一般资料、分娩方式和产褥感染情况。结果三组年龄、产次、孕周比较,差异无统计学意义(均P>0.05)。三组分娩方式及产褥感染发生率比较,差异有统计学意义(均P<0.01),A组阴道分娩率最高,其产褥感染发生率显著低于B组和C组(均P<0.0125)。结论胎膜早破期待时间对产妇分娩方式和产褥感染有显著影响,期待时间越短,自然分娩率较高,产褥感染发生率越低。有效的护理措施在胎膜早破期待治疗中的应用,可降低产褥感染发生率。Objective To explore the influence of different expectant time of premature rupture of membranes（PROM）on puerperal infection for puerperants,and to put forward nursing strategies to improve the health of maternal and infant.Methods Totally 920 late pregnant women with PROM were divided into 3groups according the length from natural rupture time to placental expulsion：group A（the time was less than 12 hours,with 531cases）,group B（the time was more than or equaled 12 hours,but less than24 hours,with 307cases）and group C（the time was more than or equaled 24 hours,with 82cases）.The general information,mode of delivery and puerperal infection were compared among the 3groups.Results There were no significant differences among the three groups in age,maternal time and gestational weeks（P〉0.05 for all）.There were significant differences among the three groups in modes of delivery and puerperal infection rates（P〈0.01 for both）,vaginal delivery rate in group A was the highest,and its puerperal infection rate was significantly lower than group B and group C（P〈0.0125 for both）.Conclusion The expectant time of PROM has significant influence on modes of delivery and puerperal infection,the shorter of the expectant time,the natural delivery rate is higher and the puerperal infection rate is lower.Application of effective nursing in expectant treatment of PROM could reduce puerperal infection rate.
