摘 要:目的分析儿童呼吸道感染标本人偏肺病毒(hMPV)的分子检测与分型。方法收集836份咽拭子流感样本和442份气管吸取物,通过实时荧光RT-PCR进行hMPV核酸检测,了解不同实时荧光对hMPV核酸蛋白基因片段扩增影响,了解其序列以及基因的分型。结果 hMPV在咽拭子流感样本检出率为6.7%(56/836),气管吸取物患者检出率为6.33%(28/442)。小于1岁患儿中hMPV在气管吸取物的检出率为6.29%(25/397),咽拭子流感样本检出率则为3.04%(12/394);冬季时期,hMPV的咽拭子流感样本检出率为13.45%(37/275),气管吸取物检出率为9.19%(24/261);共感染标本hMPV的气管吸取物检出率31.43%(11/35),咽拭子流感样本检出率为17.6%(19/108),对比差异有统计学意义(P<0.05)。结论咽拭子流感样本患者的平均年龄大于气管吸取物患者;hMPV的检出率在咽拭子流感样本和气管吸取物样本中无太大差异;<1岁患儿和共感染标本中气管吸取物检出率高于咽拭子流感样本;冬季时期咽拭子流感样本检出率高于气管吸取物。从分子分型结果看,所有样本的hMPV以A2亚型居多,不同基因型其临床特征并不显著。研究结果对儿童人感染偏肺病毒感染的防控和临床治疗治疗具有积极指导作用。Objective To analyze the molecular detection and typing of hMPV of children with respiratory tract infection. Methods 836 swabs and 442 tracheal suction of children were collected and detected by real-time fluorescent RT-PCR detection method. Results The detection rate ofhMPV in swabs and tracheal suction were 6.7% (56/836) and 6.33% (28/442); in children less than 1 years old, they were 3.04% (12/394) and 6.29% (25/397). In winter and in co-infection samples, the rates of tracheal suction samples and swabs were 31.43% (11/35) and 17.6% (19/1,08), 9.19% (24/261) and 13.45% (37/275), respectively, which showed some statistically significant difference (P〈:0.05). Conclusion The hMPV detection rate in the swabs and tracheal suction samples showed no significant difference. However, in children of less than 1 year old, in winter, and in co-infection samples, some difference was showed between these two kinds of specimens, which was statistically significant (P〈0.05). The hMPV of the samples were mostly A2 subtypes, and the difference of clinical features between different genotypes were not significant. The results of the study have a positive role in the prevention and treatment of children with hMPV infection.
