摘  要:目的探讨氢饱和水是否能提高初治肺结核患者抗氧化能力、减少药物性肝损伤的发生。方法选取初治肺结核患者90例,观察组给予HRZE(异烟肼、利福平、吡嗪酰胺及乙胺丁醇)方案加饮用氢饱和水;对照组仅给予HRZE方案。观察2组不同访视点(2周、4周、6周)血清中黄嘌呤氧化酶(XOD)、丙二醛(MDA)、超氧化物歧化酶(SOD)、谷胱甘肽转移酶(GST)水平变化及药物性肝损伤的发生情况。结果 1与对照组比较,治疗后2周,观察组GST水平高于对照组(P<0.05);治疗后4周、6周,观察组XOD、MDA水平较对照组明显下降(P<0.01),而SOD、GST水平较对照组明显升高(P<0.01)。治疗后4周、6周,观察组XOD、MDA水平明显低于治疗前,SOD、GST水平明显高于治疗前(P<0.05或P<0.01)。2治疗后6周,观察组肝损伤发生率较对照组明显下降(P<0.05)。3发生肝损伤的肺结核患者,其谷丙转氨酶(ALT)水平与XOD、MDA水平呈正相关(r=0.903、0.932,P=0.000),与SOD、GST水平呈负相关(r=-0.820、-0.760,P=0.000)。结论饮用氢饱和水能提高初治肺结核患者抗氧化能力,预防药物性肝损伤。Objective To explore whether hydrogen saturated water could increase the antioxidative ability and reduce drug-induced liver injury( DILI) in patients with initial treatment of pulmonary tuberculosis. Methods A total of 90 patients with initial treatment of pulmonary tuberculosis were selected and divided into observation group and control group. Observation group was treated with HRZE protocol( isoniazid,rifampicin,pyrazinamide and ethambutoli) combined with hydrogen saturated water for drink,while control group was treated with single HRZE protocol. The changes of levels of serum xanthine oxidase( XOD),malondialdehyde( MDA),superoxide dismutase( SOD),and glutathione S-transferase( GST) at different time points( 2,4 and 6 weeks) as well as DILI conditions were observed in both groups. Results 1 Compared with control group,observation group was significantly higher than control group in GST level after 2-weeks treatment( P〈0. 05).Meanwhile,observation group was significantly lower in levels of XOD and MDA( P〈0. 0 1),but was notably higher in SOD and GST levels than controlgroup after 4- and 6- week treatment( P〈0. 01). In addition,after 4- and 6-week treatment,XOD and MDA levels were apparently lower while SOD and GST levels were significantly higher than treatment before in observation group( P〈0. 05 or P〈0. 01).(2) Observation group was significantly lower in the rate of DILI than control group after 6-week treatment( P〈0. 05).(3) Alanine aminotransferase( ALT) level was in positive association with XOD and MDA levels( r = 0. 903,0. 932,P = 0. 000) and in negative correlation with SOD and GST levels( r =- 0. 820,- 0. 760,P = 0. 000) in tuberculosis patients with liver injury. Conclusion Application of hydrogen saturated water for drink can increase the ansitoxidative ability and reduce DILI in patients with initial treatment of pulmonary tuberculosis.
