雷公藤多苷治疗糖尿病肾病的疗效及安全性评价

摘　　要：目的:探讨雷公藤多苷治疗糖尿病肾病的疗效及安全性。方法:计算机检索Corchrane图书馆随机对照实验库(CCTR)、MEDLINE、中国生物医学数据库(CBM),清华期刊全文数据库(CNKI),手工检索10余种中文文献,收集符合纳入与排除标准的文献29篇,治疗组1078例,对照组1043例共2111例糖尿病肾病患者。采用Revman5.2.1软件,对纳入的文献进行Meta分析。结果:1雷公藤治疗组降低24小时蛋白优于对照组(P<0.0001)。2雷公藤治疗组白蛋白呈下降趋势(P=0.002)。3雷公藤治疗组对肾功无明显影响。4治疗不良反应高于对照组,以肝功能损害为主(P<0.0001)。5纳入的实验存在一定的发表偏倚及方法学质量低下。结论:雷公藤多苷治疗糖尿病肾病有一定减少蛋白尿的作用,但伴随着血浆蛋白下降,对肾功无明显影响。Objective It is to evaluate the therapeutic effect andsafty of GTW on DN.Methods Randomized controlled trials （RCTs）were indentified from CCTR,MEDLINE,CNKI,CBM.More than10 kinds of chinese journal were handsearched.29 stud-ies （2111patients）which were fit the internalizing standardwere identified.Revman5.2.1sofeware was use for Meta-analyses.Re-sults ①.Compared with the conventional treatementthe tripterygium showed more effects in reduceing the 24-hour urinary protein （P〈0.0001）.②.The treatment group compared with the control group albumin decreased.③.The treatment group had no signifi-cant effect on renal function.④Adverse reactions higher than the controlgroup,mainly to liver damag.⑤There is a certain publica-tion bias andlow-qualitytrials of these studies.Conclusion Tripterygium glycosides treatment of diabetic nephropathy have a rolein re-ducing proteinuria,but accompanied by a decrease in plasma proteins,no significant effect on renal function.
