基于骨折块大小的空心拉力螺钉固定治疗Bennett骨折效果观察

摘　　要：目的 观察基于骨折块大小的空心拉力螺钉固定治疗Bennett骨折的效果。方法 以2014年3月-2015年10月在我院接受治疗的Bennett骨折患者为观察对象。根据其治疗方式分为经皮克氏针内固定组和空心拉力螺钉固定组,观察两组患者骨折愈合时间,手术前后VAS得分和腕关节功能的差异。结果 两组患者骨折愈合时间无明显差别,而空心拉力螺钉固定组患者术后VAS得分明显低于克氏针内固定组,差异具有统计学意义（P〈0.05）;两组患者治疗前腕关节屈伸活动、尺桡偏活动、旋前旋后活动、平均握力和平均捏力与健侧百分比无明显差别,治疗后,两组患者的上述指标均较治疗前增高,且空心拉力螺钉固定组增高更明显（P〈0.05）;两组患者治疗前腕关节功能评分无明显差别,治疗后,两组患者的腕关节功能评分均增高,且空心拉力螺钉固定组增高更明显（P〈0.05）。结论 基于骨折块大小的空心拉力螺钉固定对Bennett骨折有较好的治疗效果,可明显改善患者的腕关节功能。Objective To observe the effect of screw fixation in the treatment of Bennett fracture based on the size of the fracture block. Methods Patients with Bennett fractures treated in our hospital from March 2014 to October2015 were observed.According to the treatment methods, patients were divided into percutaneous Kirschner wire fixation group and cannulated screw fixation group.The fracture healing time of the two groups was observed,and the differences of VAS score and wrist function were observed before and after operation. Results The fracture healing time between the two groups had no significant difference,while the VAS score was significantly lower than that of the patients in the operation of the screw fixation group, and the difference was statistically different（P〈0.05） ;The wrist flexion and extension activities, radial and ulnar deviation, pronation supination, average grip strength and average pinch force and the healthy side percentage had no obvious difference in two groups, after treatment,the above indexes were increased and cannulated lag screw fixation group increased more obviously （P〈0.05） ;The wrist function scores of the two groups had no significant difference before treatment.Mter treatment, the index of the two groups all increased, and the increase of the hollow lag screw fixation group was more obvious （P〈0.05）. Conclusion The hollow screw fixation based on the fracture size has a good treatment effect on Bennett fracture, which can significantly improve the patient＇s wrist function.
