全腹壁整形术的临床效果及并发症

摘　　要：目的 探讨和分析全腹壁整形术的临床效果及并发症治疗。方法 回顾性分析2005年1月至2015年12月,新疆维吾尔自治区人民医院完成的38例全腹壁整形术患者的病历资料,所有患者均行下腹横“W”形切口,一期均未联合进行脂肪抽吸术,分析患者的治疗效果及术后并发症的发生、治疗情况。结果 术后1~2年随访到30例全腹壁整形术患者,经过手术治疗后腹壁皮肤松弛、下垂均得到了明显改善,患者腹壁平坦,皮肤松垂消失,塑形效果良好,无肺梗死、肺栓塞、下肢深静脉栓塞等重大并发症发生。其中术后1例患者发生切口轻度感染;1例出现皮瓣远端切口处小面积皮肤缺血、坏死;1例出现皮下血肿,经及时对症处理后伤口均愈合,其余27例患者术后切口均一期愈合。结论 全腹壁整形术是一种去除腹壁中、下部过多皮肤、脂肪,并将腹壁肌肉收紧的整形外科手术,对于腹壁塑形,具有高效、外形佳、立竿见影等优点,只要仔细筛选患者,全腹壁整形术不失为一种最有效的腹壁整形方法。Objective To investigate and analyze the clinical effect and complications of abdominoplasty. Methods From January 2005 to December 2015,the medical records of patients underwent abdominoplasty in Xinjiang People＇s Hospital were collectted. A total of 38 cases were enrolled. Of these, all patients underwent ＂W＂ shaped incision in lower abdomen, and none of them received the simultaneous liposuction. The clinical outcomes and occurrence of postoperative complications were analyzed. Results After 1 to 2 years, 30 patients were followed up. After surgical treatment, the abdominal skin laxity and sagging had been significantly improved. Patientg abdominal wall was flat, the sagging skin was disappeared, and the shaping effect was good. No serious complications such as pulmonary infarction, pulmonary embolism or deep vein thrombosis occurred after surgery. Mild incision infection occurred in one patient; one patient had a small area skin ischemia and necrosis in distal flap, and another one developed a subcutaneous hematoma, which healed after asymptomatic treatment. The remaining 27 patients had recovered uneventfully. Conclusion Abdominoplasty is a plastic surgery which can remove excessive skin and fat of middle and lower abdominal wall, and can tight abdominal muscle for abdominal shaping. It has the advantages of high efficiency, good cosmetic effect and immediate benefit. As long as careful screening of patients, the whole abdominoplasty can be regarded as one of the most effective methods of abdominal plastic surgery.
