异丙酚静脉全身麻醉在老年无痛内镜检查中的效果分析

摘　　要：目的分析异丙酚静脉全身麻醉在老年无痛内镜检查中的效果。方法选取我院2016年2月至2016年10月收治的需行无痛膀胱镜检查的患者60例,随机分为A组和B组,每组各30例,A组患者采取静脉注射异丙酚进行麻醉,B组患者采取异丙酚复合瑞芬太尼进行麻醉。监测并比较两组患者平均动脉压(MAP)、心率(HR)、血氧饱和度(SpO_2)、异丙酚首剂量、意识消失时间、苏醒时间和不良反应发生情况。结果 A组患者麻醉过程中SpO_2与基础值比较无明显变化(P>0.05),但MAP和HR均降低(P<0.05),差异具有统计学意义。B组患者麻醉过程中MAP、HR、SpO_2与基础值比较,均未见明显变化,P>0.05,差异无统计学意义。B组患者的首剂量和意识消失时间都明显低于A组患者,P<0.05,差异具有统计学意义。两组患者的苏醒时间比较无明显差异(P>0.05)结论异丙酚复合瑞芬太尼静脉全身麻醉用于老年无痛膀胱镜检查具有良好的麻醉效果和镇痛效果,利于患者术后恢复,值得临床上推广应用。Objective To analyze the anesthesia effect of intravenous propofol general for elderly painless endoscopy. Methods Selected 60 patients for painless endoscop in our hospital during February 2016 to October 2016, randomly divided them into group A and group B, 30 cases in each group. Patients of group A received intravenous propofol anesthesia, patients of group B received propofol compound remifentanil intravenous general anesthesia. Monitored and compared patients of the two groups with mean arterial pressure （MAP）, heart rate （HR）, blood oxygen saturation （SpO2）, the first dose of propofol, unconsciousness time, awakening time and adverse reactions occur. Results The SpO2 of group A had no significant difference compared with basic value（P〉0.05）, but the MAP and HR were decreased（P〈0.05）, the difference is statistically significant. MAP, HR, SpO2 of group B had no significant change compared with basic value during anesthesia（P〉0.05）. The first dose and disappear time of group B were significant lower than those of group A（P〈0.05）. The recovery time of the two groups had no significant difference（P〉0.05）： Conclusion Propofol compound remifentanil intravenous general anesthesia on elderly painless cystoscopy has good effect of anesthesia and analgesia effect, good for patients with postoperative recovery, worthy of clinical application.
