摘 要:目的对临床药师用药咨询门诊工作效果进行分析,为探索建立有效用药咨询门诊提供参考。方法收集临床药师用药咨询门诊的咨询记录,筛选有效用药咨询记录,对其进行统计与分析。结果共纳入有效咨询记录172条。咨询者中,患者96例,占55.82%;医生38例,占22.09%;患者家属29例,占16.86%;护士8例,占4.65%;药师1例,占0.58%。咨询者主要来自肿瘤化疗科32例(占18.60%)、肾脏内科和心血管内科分别为18例(占10.47%)。咨询方式中,现场咨询161例,占93.60%;电话咨询11例,占6.40%。咨询的药物共涉及12类,主要包括作用于中枢神经系统药物(35例)、激素类药物(26例)和抗感染药物(23例)。咨询的内容以药物的用法用量为主,共计56例;其他分别为药物的不良反应、药物的基本信息等。临床药师在用药咨询门诊解答患者用药咨询时主要依据自身的专业知识,共计123例;其他依次为书籍查询、药品说明书和网络查询。结论开设临床药师用药咨询门诊可为咨询者及时、有效地解决用药过程中遇到的问题,但其运行模式有待进一步完善,咨询内容仍需进一步加强宣传,以便更好地为医务人员和患者提供有效用药咨询。Objective To analyze the performance of clinical pharmacist medication consultation in outpatient department,so as to establish effective drug consultation in outpatient department. Methods We collected and screened consultation records for statistics and analysis. Results There were 172 effective consultation records. Among consultants,there were 96 cases( 55. 82%) of patients,38 cases( 22. 09%) of doctors,29 cases( 16. 86%) of patients' family members,8 cases( 4. 65%) of nurses,and 1 case( 0. 58%) of pharmacist. There were 32 cases( 18. 60%) of consultants from tumor chemotherapy department,18 cases( 10. 47%) from renal department and internal medicine of cardiovascular department. As for consulting method,there were 161 cases( 93. 60%) of on-site consultations,and 11 cases( 6. 40%) of telephone consultations. The consulted drugs involved 12 classes,including central nervous system drugs( 35 cases),hormone drugs( 26 cases),and anti-infection drugs( 23 cases). A total of 56 cases of consultation was related to the usage and dosage of drugs,followed by drug adverse reactions and basic information of drugs. The clinical pharmacists mainly drew on their professional knowledge to answer consultations,followed by books,dispensatory and network. Conclusions Established clinical pharmacist medication consultation can timely and effectively solve the problems during the treatment of patients,but its operation mode needs to be improved and consulting contents need to be further publicized,in order to provide more effective medication consultation for medical staffs and patients.
