早期下床活动对泌尿外科腹腔镜手术患者快速康复的影响

摘　　要：目的探索和实践早期下床活动对泌尿外科腹腔镜手术患者快速康复的影响。方法将124例行腹腔镜手术治疗的患者按照随机数字表法分为对照组和观察组,每组62例,对照组采用常规围术期护理,观察组进行早期下床活动干预,术后记录患者下床活动情况,观察早期下床活动对患者进食时间(h)、输液时间(d)、术后住院时间(d)、并发症、住院费用(元)的影响。结果观察组与对照组相比,术后首次进食时间观察组为(8.4±3.7)h,对照组为(17.5±3.4)h;静脉输液时间观察组为(5.5±1.7)d,对照组为(6.7±1.3)d;术后住院天数观察组为(6.2±1.7)d,对照组为(7.5±1.4)d;治疗总费用观察组为(2.0±0.5)万元,对照组为(2.4±0.6)万元,患者进食时间提前,输液时间、术后住院时间缩短,并发症发生率减少,住院费用降低,以上指标两组比较,差异有统计学意义(P<0.05)。结论对泌尿外科腹腔镜手术患者进行早期下床活动的护理干预可以减少术后住院时间,减少住院费用,促进患者快速康复。Objective To investigate the effects of early ambulation on fast track in patients undergoing urogenital laparoscopic surgeries. Methods Patients who underwent urogenital laparoscopic surgery in Department of Urology,The First Affiliated Hospital of Anhui Medical University,were randomized into control group and observation group,with 62 patients in each group. Patients in the control group had conventional preoperative nursing,while early ambulation intervention was taken in the observation group. Subsequently,ambulation was documented in both groups,as well as the effects of early ambulation on eating time（ d）,venous transfusion time（ d）,postoperative hospital stays（ d）,complications and hospitalization costs. Results Postoperative feeding time for the first time was（ 8. 4 ±3. 7） h in the observation group and（ 17. 5 ± 3. 4） h in the control group; intravenous infusion time was（ 5. 5 ± 1. 7） d in the observation group,and（ 6. 7 ± 1. 3） d in the control group; postoperative hospitalization was（ 6. 2 ± 1. 7） d in the observation group,and（ 7. 5 ± 1. 4） d in the control group; the total cost of treatment was（ 2. 0 ± 0. 5） ten thousand yuan in the observation group,and（ 2. 4 ± 0. 6） ten thousand yuan in the the control group. There were significant differences in the above indexes between the two groups（ P〈0. 05）. Conclusions Nursing interventions with early ambulation in patients after urogenital laparoscopic surgery can shorten postoperative hospital stay,reduce hospitalization cost and promote rapid recovery.
