地特胰岛素联合二甲双胍治疗2型糖尿病的临床观察

摘　　要：目的观察未应用过胰岛素治疗、单纯口服二甲双胍血糖未能达标的2型糖尿病患者,启用每日1次地特胰岛素治疗的效果及安全性。方法选取初诊的2型糖尿病患者60例随机分为地特胰岛素联合二甲双胍(治疗组)及格列美脲联合二甲双胍(对照组),比较2组治疗前后糖化血红蛋白、7个时点的血糖、体质量变化,以及治疗期间低血糖事件和过敏反应情况。结果治疗后2组糖化血红蛋白及7个时点的血糖均显著低于治疗前(P<0.05),且治疗组低于对照组(P<0.05)。2组体质量组间和治疗前后比较差异均无统计学意义(P>0.05)。2组均未出现低血糖事件及过敏反应。结论地特胰岛素联合二甲双胍治疗2型糖尿病,能有效控制血糖,且不增加患者体质量,也不增加低血糖事件及过敏反应发生率。Objective To observe the efficacy and safety of initiating insulin therapy with insulin detemir in oral antidiabetic drug metformin-treated patients with type 2diabetes mellitus（T2DM）whose blood glucose is not yet up to the standard.Methods Sixty subjects with T2 DM were randomly divided into two groups,insulin determir combining with metformin（treatment group）and glimepriride combining with metformin（conrtol group）.This treatment ruly monitored hemoglobin A1 c,7-point blood glucose,body weight,hypoglycemic event,allergic reaction.During the 32-week-treatment,the dosages of medicine were adjusted according to selfrecorded daily 7-point blood glucose.This clinical observation is to evaluate the efficacy and safety of insulin determir.Results The 7-point blood glucose of the 2groups after treatment were significantly lower than those before treatmen（P〈0.05）.The treatment group was lower than the control group（P〈0.05）,but there′s no significant difference（P〉0.05）.There was no hypoglycemic event or allergic reaction during the treatment.Conclusion Insulin detemir combined with metformin certainly reduce blood glucose in T2 DM patients effectively.Insulin detemir combined with metformin does not increase the patient′s body mass,nor does it increase the incidence of hypoglycemic events and allergic reactions.
