杂交手术治疗颅内动脉瘤的研究

摘　　要：目的：探讨运用杂交手术治疗颅内动脉瘤的价值.方 法：15例颅内动脉瘤患者术前行CTA明确有动脉瘤后,在杂交手术室先行DSA造影进一步明确动脉瘤位置、大小、类型,然后行颅内动脉瘤夹闭,完成夹闭后再次DSA造影检查,评估夹闭情况,决定是否需要调整动脉瘤夹.结果：15例动脉瘤中,1例误夹脉络膜前动脉,2例夹闭不全.结论：颅动脉瘤夹闭术中联合DSA造影,能及时、安全、有效的指导动脉瘤的夹闭,预防误夹或者夹闭不全等情况发生,尤其是在瘤颈不宜暴露或者暴露不充分的情况下,尤为有用.Objective：The discussion of the value about hybrid operation.Methods 15 patients with intracranial aneurysms did computed tomography angiography（CTA） to determine, and did digital subtractio n angiography（DSA） to ensure the location, size and type of intracranial aneurysms.Then clip the aneurysm und er the Craniotomy Surgery. DSA imaging examination again,evaluation of clip, decide whether to need to adjust the aneurysm clip.Results 1 case was mistakenly clip the anterior choroidea,2 cases of clip is not completely.Conclusions hybrid operation of intracranial aneurysm can timely,safe and effective guidance of aneurysm cl ip.It can prevent the mistake of clip, especially in tumor neck expose dissatisfactorily.
