摘 要:研究背景及目的: 农民石工尘肺在四川农村是一种常见的肺部疾病.本文探讨其非工业性职业肺部疾病的相关流行病学情况.方法:2014 年10月通过采集农民石工的作业史和胸部电视 X线透视、DR及CT检查分层筛查,对比分析同时完成DR及CT胸片的影像改变,并测定部分受检者的肺功能.结果:全部受检者 344 人,经电视X线胸部透视者176人,胸部DR片检查者168人,男164人,女4人,年龄平均 56.51岁(35-84岁),DR片有双肺弥漫结节影21 例,可疑结节影 20 例及肺气肿、肺结核等不同异常表现者47例,经胸部CT 检查25例,对比25例同时DR及CT检查诊断符合尘肺者 12 例,可疑尘肺 4例,其他肺部疾病改变着6例,CT 检查显著优于DR胸片(p〈0.05).肺功能测定结果示,混合性通气功能障碍 14例(10.94%),阻塞性通气功能障碍 49例(38.28%),限制性通气功能受损 8例(6.25%).结论:农民石工尘肺及其肺功能受损值得重视;胸部CT诊断尘肺显著优于胸部DR检查.利用X线电视透视、DR胸片和胸部CT检查进行逐层筛选,对肺部疾病人群流行病学调查和诊断是有效方法.Background and Aim: farmer’s stonemason pneumoconiosis (FSP) is a common pulmonary disease in the countryside of Sichuan. The paper aim know relative epidemiological data of non-industrial occupational pulmonary diseases in the farmer’s stonemasons. Methods: the working experience of farmer’s stonemasons, and their video X-ray chest fluoroscopy, DR chest portrait and chest CT were checked according to leyer, and chest DR and CT were checked in-step and contrasted in 25 of farmer’s stonemasons, and majority of subjects of pulmonary functions were tested. Results: All subjects are 344 persons, 176 of subjects are video X-ray chest fluoroscopy, 168 are DR chest portrait, 25 aer chest CT. 21 cases had suffuse pulmonary node in chest DR film, 20 cases had suspicious sporadic pulmonary node, and 47 cases of pulmonary emphysema, tuberculosis and other various pulmonary expression. 25 cases of comparisons to DR and CT film shows 12 cases meet criterion of pneumoconiosis, 4 cases are suspicious, 6 cases had other pulmonary diseases, CT of clinical accuracy higher than chest DR by 25 cases of chest film checked(p〈0.05). The outcomings of pulmonary functions show barrier of obstructive ventilation function is 38.28%, mixing ventilation function is 10.94%,and limited ventilation function 6.25%. Conclusions: The damages of lungs in FSP and wasted pulmonary function is worthy of attention. The accuracy by chest CT of diagnosed pneumoconiosis markedly better than chest DR. It is a effective way to diagnosis and epidemiological survey of population of lung disease, using X-ray TV, chest DR film and chest CT examination of a csas-by-layer filtering.
