非梗死心电图及超声心动图对缺血相关冠状动脉的价值分析

摘　　要：目的：通过研究冠状动脉造影结果,探讨非梗死心电图及超声心动图结果对缺血相关冠状动脉的诊断价值.方法：选取我院诊断为冠心病、不稳定型心绞痛患者2658例,并行冠状动脉造影术,根据造影结果对心电图及超声心动图结果进行对照分析.结果：非梗死心电图及超声心动图对缺血相关冠状动脉的特异性及阳性预测值高,特异性分别为67.76%,85.37%,阳性预测值分别为： 90.33%、93.65%,但敏感性低,分别为： 43.47%,31.12%.当冠脉狭窄50～70%时非梗死心电图及超声心动图的阴性预测值分别为：65.79%、88.35%,狭窄71%～90%时阴性预测值分别为：53.98%、60.88%,当狭窄〉90%时阴性预测值分别为：46.28%,60.79%,表明超声心动图对冠状动脉狭窄程度的预测价值高于非梗死心电图.结论：冠状动脉病变与非梗死心电图及超声心动图对冠状动脉对病变有价值.ObjectiveAnalyzing the correlations among no-infarction electrocardiogram （ECG）、 echocardiography and Coronary artery ischemia. Method： Conducting coronary arteriography for 2658 patients diagnosed asCoronary heart disease, unstable angina pectoris in our hospital, and analyzing the outcoming of coronary arteriography, the ECG and echocardiography.Result： Non-infarction ECG, echocardiography’s predictive values of the specificity and positive of ischemia related coronary artery are high. The specificities are 67.76%,85.37% and the positive values are 90.33%, 93.65%. On the other hand, the sensitivity are 43.47%,31.12% respectively, which are low. The negative predictive value of Non-infarction ECG, echocardiography are 65.79%、88.35%; 53.98%、60.88% and 46.28%,60.79% respectively when coronary artery stenosis are 50～ 70%、71%～90% and over 90%. Non-infarction ECG’s values of predicting Coronary artery stenosis is lower than that of echocardiography.Conclusion： There are correlations among Non-infarction ECG, echocardiography and Coronary artery disease, which is valuable in forecasting of the severity of coronary artery stenosis.
