摘 要:目的 探讨老年脑梗死患者颈动脉硬化与血浆脂蛋白磷脂酶 A2(Lipoprotein phospholipase A2,Lp-PLA2)含量的关系. 方法 抽取111例老年脑梗死患者作为研究对象,选取同期健康体检人群109例作为对照组, 采用上转发光法测定其血浆 Lp-PLA2含量.给予老年脑梗死患者彩色多普勒超声检查,按照两侧颈动脉硬化的具体状况,分为颈动脉内膜中层厚度( Intima-media thickness, IMT)正常组、IMT 增厚组、颈动脉稳定斑块组、易损斑块组和颈动脉狭窄组5个亚组,测定并比较各组血浆 Lp-PLA2含量. 结果 Spearman相关性分析显示 Lp-PLA2水平与颈动脉硬化呈正相关(r=0.751,P〈0.01).脑梗死组 Lp-PLA2明显高于对照组(P〈0.01),差异有统计学意义.在老年脑梗死患者各亚组中,颈动脉 IMT 增厚组 Lp-PLA2与 IMT 正常组比较,差异无统计学意义(P〉0.05);狭窄组 Lp-PLA 2与易损斑块组比较,差异无统计学意义(P〉0.05);余各亚组间 Lp-PLA2水平比较均有统计学意义(P〈0.01,P〈0.05).结论 老年脑梗死患者颈动脉硬化与 Lp-PLA2有一定的相关性,临床可通过检测 Lp-PLA2含量来判断其炎症程度和斑块易损度,更准确评估脑梗死复发风险.Aim To study the relationship between senile cerebral infarction with carotid atherosclerosis and plasma lipoprotein phospholipase A2 levels. Methods 111 patients with senile cerebral infarction were selected as the research object and 60 cases of healthwere check-up crowd from the same time period were selected as control group. Forward light approach is used to determine the content of plasma Lp-PLA2. According to the two side of carotid atherosclerosis, senile cerebral infarction patients were divided into 5 subgroups, including carotid IMT normal group, IMT thickening group, stable carotid plaque group, vulnerable plaque group and carotid stenosis group,and the plasma Lp-PLA2 content of every group were deteced and compared. Results Pearson correlation analysis showed positive correlation between carotid arteriosclerosis and the level of Lp-PLA2(r=0.926, P〈0.01).Lp-PLA2 of cerebral infarction group was obviously higher than that of control group(P〈0.01), and the difference was statistically significant.In all the subgroups,there was no statistically significant difference (P〉0.05) between the carotid IMT thickening group and normal IMT group. Between the narrow set of Lp-PLA2 and the vulnerable plaques group, there was no statistically significant difference (P〉0.05).Among the other subgroups,there were statistically significant differences (P〈0.01, P〈0.05). Conclusion Carotid atherosclerosis of the senile cerebral infarction patients has the certain correlation with the level of Lp-PLA2. Through the detection of Lp-PLA2 to determine the degree of inflammation and plaque vulnerability degree in clinical, a more accurate assessment of cerebral infarction risk of recurrence.
