复方丹参片联合低分子肝素治疗对早发型重度子痫前期围产结局的影响

摘　　要：目的研究临床使用复方丹参片和低分子肝素对早发型重度子痫前期围产结局的影响。方法选取广东省妇幼保健院2013年10月至2015年10月收治的86例早发型重度子痫前期患者为研究对象,按照随机数字表法分为A组(48例)和B组(38例),两组患者均给予常规治疗,其中A组患者在常规治疗的基础上增加复方丹参片,B组患者在常规治疗的基础上联合使用低分子肝素和复方丹参片治疗,对比分析两组患者治疗效果及围产结局情况。结果两组患者围产结局中有不同程度的低蛋白血症、肝肾功能衰竭、视物障碍、胸腹水等并发症,B组患者并发症发生率(36.84%)低于A组(52.08%),两组患者均出现不同程度的胎儿宫内窘迫、新生儿窒息等症状,B组新生儿并发症发生率(34.21%)低于A组(45.83%),差异有统计学意义(P<0.05);其中B组新生儿1min和5min Apgar评分优于A组(P<0.05)。结论复方丹参片和低分子肝素联合使用能够有效改善早发型重度子痫前期患者围产结局,具有一定临床治疗价值。Objective To explore the effect of compound salvia tablet and low molecular heparin on pefinatal outcome of early onset severe preeclampsia. Methods 86 patients with early onset severe preeclampsia in Maternal and Child Health Care Hospital of Guangclong Province from October 2013 to October 2015 were selected. They were divided into group A （n = 48） and group B （n = 38） by random number table. Patients of two groups were given conventional treatment, patients in group A were given compound salvia tablet, and patients in group B were given compound salvia tablet and low molecular heparin. Prognosis and complications of mothers and babies of two groups were compared. Results Patients of two groups had different degrees of hypoalbuminemia, liver and kidney function failure, vision disorders, hydrothorax, ascites and other complications after treatmen. The occurrence rate of complications of group B （36. 84% ） was significantly lower than group A （52. 08% ）, different degree of fetal distress, neonatal distress and other symptoms occured in two groups. The incidence rate of neonatal complications （34. 21% ） in group B was lower than that in group A （45. 83% ）, the differences were statistically significant （P 〈0. 05）. The newborn 1 win and 5 min Apgar score of group B were significantly higher than that of group A （ P 〈 0. 05 ）. Conclusion The combination of compound salvia tablet and low molecular heparin can effectively improve the perinatal outcome of early onset severe preeclampsia patients, and have positive clinical significance.
