从痿论治咽喉反流性疾病

摘　　要：咽喉反流性疾病根本在于脾不主肌,失其约弛,气机逆乱所致,故临证中以痿论治,遵"治痿独取阳明"之法,其本为脾胃虚弱,肌膜失养,痿废不用,气机逆乱,升降失常,"形不足者,温之于气",遵李东垣之补中益气汤加减,治以"补益脾胃,调畅气机"之法,兼以化痰祛瘀,清利咽喉积滞。"治痿独取阳明"中的"阳明",不应单纯认为是阳明脏腑,亦应从阳明经脉的角度治疗,故近部选穴取足阳明胃经之人迎穴,水突穴,手阳明大肠经之扶突穴针刺以润咽喉,调气机,结合辨经取穴,取足阳明胃经及足太阴脾经之荥穴内庭和大都用补法以"移气于不足"。The etiology of laryngopharyngeal reflux disease （LPRD） lies in the spleen failing to govern muscles causing disorder of qi activity,so in clinical practice,treating LPRD should beginning from treating flaccidity by tollowing the method of yangming se- lected to treat flaccidity,because its pathogenesis is weakness of the spleen and stomach,loss of nutrients of muscles and mem- brane, uselessness due to flaccidity, disorder of qi activity, abnormal ascent and descent. Lack of form should be treated by war- ruing qi,so according to Li Dong-：huan＇s Modified Center-Supplementing and Qi-Boosting Decoction, therapy of nourishing the spleen and stomach, and regulating qi activity is adopted to resolve phlegm and remove blood stasis, anti clear away throat stagna- tion, ＂Yangming＂ in ＂yangming selected to treat flaccidity＂ not only means yangming viscera, but also means yangming meridian. Therefore, in terms of selecting the points in the local area, Renying （ ST 9 ） , Shuitu （ ST 10 ） of the stomach meridian of foot- yangming, Futu （LI 18 ） of the large intestine meridian of hang-yangming are selected for acupuncture to moisten throat,regulate qi activity. Combining selecting points based on meridian differentiation, Neining （ST 44 ） of the stomach meridian of foot-yang- ruing, and Dadu （ SP 2 ） of the spleen meridian of foot-taiyin are selected to nourish the lack of body.
