3种常见肺部真菌感染CT表现的比较研究

摘　　要：目的比较分析3种肺部真菌感染(肺念珠菌病、侵入性肺曲霉病、肺隐球菌病)CT表现,为临床治疗提供参考依据。方法回顾性分析2011年7月-2015年7月医院确诊82例肺部真菌感染患者临床资料,其中21例肺念珠菌病、41例侵入性肺曲霉病(IPA)、20例肺隐球菌病患者,比较3种常见肺部真菌感染CT表现。结果肺隐球菌病患者单纯性病变显著高于肺念珠菌病以及IPA患者(P<0.05),肺隐球菌病患者聚集性结节、支气管充气征发生率显著高于肺念珠菌病以及IPA患者(P<0.05),IPA空洞发生率高于肺念珠菌病、肺隐球菌病患者(P<0.05)。结论肺隐球菌病结节主要以聚集型分布、支气管充气征为特点,而IPA结节则以空洞尤其是空气半月征为特点,肺念珠菌病磨玻璃影发生率最高。OBJECTIVE To compare and analyze the CT manifestations of three kinds of pulmonary fungal infections（pulmonary candidiasis,invasive pulmonary aspergillosis,pulmonary cryptococcosis）,so as to provide evidence for clinical treatment.METHODS The clinical data of 82 patients with pulmonary fungal infection diagnosed from Jul.2011 to Jul.2015 were retrospectively analyzed,including 21 cases of pulmonary candidiasis,41 cases of invasive pulmonary aspergillosis（IPA）,and 20 cases of pulmonary cryptococcosis.The CT manifestations of three kinds of common pulmonary fungal infections were compared.RESULTS Simple lesions in patients with pulmonary cryptococcosis were significantly higher than those of pulmonary candidiasis and IPA patients（P〈0.05）,the occurrence rates of gathered nodular and air bronchus syndrome in patients with pulmonary cryptococcosis were significantly higher than those of pulmonary candidiasis and IPA patients（P〈0.05）,and voids occurrence rate in patients with IPA was higher than that of pulmonary candidiasis and pulmonary cryptococcosis patients（P〈0.05）.CONCLUSIONPulmonary cryptococcosis nodules have the characters of aggregated distribution and air bronchogram,while the IPA nodules are hollow,especially the characteristics of air crescent sign,and pulmonary candidiasis has the highest incidence of ground glass shadow.
