摘　　要：目的探讨膜式液基薄层细胞学技术(TCT)及高危型人乳头状瘤病毒(HPV)检查联合阴道镜下宫颈活检在宫颈病变中的临床应用价值。方法选取2013年1月-2015年11月接受宫颈疾病筛查的5 028例已婚女性为研究对象,对所有纳入研究的受试者行宫颈TCT、HPV检查后再行阴道镜下宫颈活检,对比两次检查结果,以病理学检查结果为诊断标准,判断两种检查方法对宫颈相关疾病的检出率,得出两种方法检测的特异性、灵敏度和准确性。结果宫颈TCT、HPV检查阳性患者为736例(14.64%),TCT、HPV检查联合阴道镜下宫颈活检阳性患者为1 025例(20.39%),两组检出率比较差异有统计学意义(P<0.05)。两种方法的特异性、灵敏度和准确性比较差异有统计学意义(P<0.05)。结论 TCT、HPV检查联合阴道镜下宫颈活检对宫颈疾病的临床检出率较高。宫颈TCT、HPV筛查出现高危情况的患者进一步行阴道镜下宫颈活检可以提高宫颈疾病的检出率,逐层递进检查可以降低筛选成本。Objective To explore the clinical application value of thinprep cytology test（ TCT） and high- risk human papillomavirus（HPV） test combined with colposcopic cervical biopsy in cervical lesions. Methods A total of 5 028 married women receiving cervical diseases screening in the hospital from January 2013 to November 2015 were selected,then TCT,HPV test,and colposcopic cervical biopsy were performed. The detection results were compared,the result of pathological examination was the gold standard,the detection rates of the two methods for cervical diseases were determined,the specificity,sensitivity and accuracy were calculated. Results The positive rate of TCT and HPV test was 14. 64%（736 cases）,and the positive rate of TCT,HPV combined with colposcopic cervical biopsy was 20. 39%（1 025 cases）,there was statistically significant difference between the two groups（P〈0. 05）. There was statistically significant difference in specificity,sensitivity and accuracy between the two methods（ P〈0. 05）. Conclusion The clinical detection rate of TCT,HPV combined with colposcopic cervical biopsy is high for detecting cervical diseases. Colposcopic cervical biopsy can improve the detection rate of cervical diseases among the patients with high risk results of TCT and HPV test,layer by layer progressive examiantion can reduce the costof screening.
