未足月胎膜早破孕妇的易发因素及对妊娠结局的影响

摘　　要：目的研究未足月胎膜早破(PPROM)孕妇的易发因素及对妊娠结局的影响,为临床治疗提供依据。方法选取2013年11月-2014年2月该院收治的胎膜早破(PROM)孕妇150例,根据是否足月将孕妇分为研究组(未足月80例)和对照组(足月70例),分析PPROM的易发因素,并观察其对妊娠结局的影响。结果研究组人工流产≥2次、胎位异常及生殖道感染的发生率显著高于对照组,差异具有统计学意义(P<0.05);多因素分析显示:人工流产≥2次、胎位异常及生殖道感染是导致PPROM的危险因素(P<0.05);研究组新生儿感染、窒息及胎儿窘迫发生率显著高于对照组,差异具有统计学意义(P<0.05);两组产妇产后出血、剖宫产及产褥感染发生率比较,差异均无统计学意义(P>0.05)。结论 PPROM与人工流产≥2次、胎位异常及生殖道感染有关,且PPROM对胎儿影响较大。Objective To research the predisposing factors of preterm premature rupture of fetal membrane（ PPROM） and its effect on pregnancy outcome,provide a basis for clinical treatment. Methods A total of 150 PPROM pregnant women treated in the hospital from November 2013 to February 2014 were selected,then they were divided into study group（80 premature infants） and control group（70 full-term infants）,the predisposing factors of PPROM were analyzed,the effect of PPROM on pregnancy outcome was observed. Results The incidence rates of abortion≥2 times,abnormal fetal position,and genital tract infection in study group were statistically significantly higher than those in control group（ P〈0.05）; multivariate analysis showed that abortion≥2 times,abnormal fetal position,and genital tract infection were risk factors of PPROM（ P〈0.05）; the incidence rates of neonatal infection,asphyxia,and fetal distress in study group were statistically significantly higher than those in control group（ P〈0.05）; there was no statistically significant difference in the incidence rates of postpartum hemorrhage,cesarean section,and puerperal infection between the two groups（ P〈0.05）. Conclusion PPROM is related to abortion≥2 times,abnormal fetal position,and genital tract infection,PPROM has a great impact on fetuses.
