经典Macdonald术与U形加固宫颈环扎术治疗宫颈机能不全的对照研究

摘　　要：目的对比分析经典Macdonald宫颈环扎术与U形加固宫颈环扎术治疗宫颈机能不全的疗效。方法回顾分析2010年1月-2015年7月该院收治的83例宫颈机能不全孕妇的临床资料,根据手术方式分为观察组(43例)与对照组(40例),观察组患者接受经典Macdonald宫颈环扎术治疗,对照组患者接受U形加固宫颈环扎术治疗。治疗结束后,所有孕妇均接受随访,直至妊娠结束。结果 1两组孕妇晚期流产率、早产率、足月分娩率相比差异无统计学意义(P>0.05)。2两组产妇分娩方式相比差异无统计学意义(P>0.05)。3两组新生儿窒息发生率、新生儿住院率相比差异无统计学意义(P>0.05)。结论宫颈环扎术治疗宫颈机能不全的疗效确切,经典Macdonald宫颈环扎术与U形加固宫颈环扎术的疗效相当。Objective To compare and analyze the curative effects of classical Macdonald operation and cervical cerclage reinforced by U- jacket in treatment of cervical incompetence. Methods The clinical data of 83 pregnant women with cervical incompetence from the hospital from January 2010 to July 2015 was analyzed retrospectively,then the pregnant women were divided into observation group（43 cases） and control group（40 cases）. The cases in observation group were treated by classical Macdonald operation,and the cases in control group were treated by cervical cerclage reinforced by U- jacket. After the treatment,all the cases were followed up until the end of pregnancy. Results There was no statistically significant difference in the rates of late abortion,premature labor,and full- term delivery between the two groups（ P〉0. 05）. There was no statistically significant difference in delivery modes between the two groups（ P〉0. 05）. There was no statistically significant difference in the incidence rate of neonatal asphyxia and the hospitalization rate of neonates between the two groups（ P〉0. 05）. Conclusion The curative effect of cervical cerclage in treatment of cervical incompetence is exact. The curative effects of classical Macdonald operation and cervical cerclage reinforced by U- jacket are similar.
