探讨B-Lynch缝合术治疗剖宫产产后出血的临床疗效及术后出血量的变化

摘　　要：目的探讨B-Lynch缝合术治疗剖宫产产后出血的临床疗效及术后出血量的变化。方法回顾分析2014年9月-2016年8月医院收治的剖宫产产后出血患者96例,按照患者产后出血处理方式将其分为观察组和对照组,每组各48例,观察组患者实施B-Lynch缝合术治疗,对照组患者按照常规血窦缝合法加用宫腔纱布填塞止血;对比两组患者的临床疗效,并采取容积法评估患者术后出血量改善情况。结果观察组患者在行产后止血治疗24 h内出血量、止血操作耗时、恶露干净时间以及月经来潮时间均明显低于对照组,差异有统计学意义(P<0.01);术后Hb指标明显高于对照组,术后恢复期输血例数与感染、产褥等并发症率均明显低于对照组,差异均有统计学意义(P<0.01)。结论 B-Lynch缝合术治疗剖宫产产后出血疗效确切,可在短时间内控制产妇出血量,改善宫腔肌层收缩力,促进止血效果提升,降低术后并发症风险,保证患者预后恢复质量,值得推广应用。Objective To explore the clinical effect of B- Lynch suture in treatment of postpartum hemorrhage after cesarean section and the change of the amount of bleeding after operation. Methods Ninety- six patients with postpartum hemorrhage after cesarean section were selected from the hospital from September 2014 to August 2016,then the clinical data was analyzed retrospectively,all the patients were divided into observation group and control group,48 patients in each group. The patients in observation group were treated by B- Lynch suture treatment,and the patients in control group were treated by conventional blood sinus suture combined with intrauterine gauze packing;the clinical effects in the two groups were compared,volumetric method was used to evaluate the improvement of amount of postoperative bleeding. Results The amount of bleeding within 24 hours,the hemostatic operation time,the disappearance time of lochia,and menstrual time in observation group were statistically significantly lower than those in control group（ P 〈0. 01）; Hb level after operation in observation group was significantly higher than that in control group,the number of blood transfusion during postoperative recovery period and the incidence rates of infection and puerperal complications in observation group were statistically significantly lower than those in control group（ P 〈0. 01）. Conclusion The curative effect of B- Lynch suture in treatment of postpartum hemorrhage after cesarean section is exact,B-Lynch suture cam control maternal bleeding in a short time,improve uterine myometrial contractility,promote the hemostatic effect,reduce the risk of postoperative complications,ensure the quality of recovery,which is worthy of popularization and application.
