摘  要:目的探讨超声造影(CEUS)成像时间-强度曲线(TIC)新指标对甲状腺良恶性结节的诊断价值。方法回顾性分析67个甲状腺结节造影成像TIC曲线,记录结节内感兴趣区(ROI)峰值强度(IMAX)、始增时间(RT)、达峰时间(TTP),以周围甲状腺组织ROI为基准,得到病灶相对IMAX(△Ι)、相对RT(△RT)、相对TTP(△TTP)。结果甲状腺良性结节IMAX、△Ι(142.6±55.6,46.3±55.4)明显大于恶性结节(75.2±34.7,-17.7±34.9)(P<0.05),△RT、△TTP(-0.3±1.7,-0.4±1.6)明显小于恶性(<1cm)结节(0.8±1.9,0.7±1.8)(P<0.05)。恶性(>1cm)结节△RT(-0.4±1.3)明显小于恶性(<1cm)结节(0.8±1.9)(P<0.05)。结论超声造影成像有助于甲状腺结节良恶性的鉴别诊断。Objective To assess the value of new parameters of the time-intensity curve(TIC) of contrast-en- hanced ultrasound(CEUS) in differential diagnosis of benign and malignant thyroid nodules. Methods 67 thyroid nod- ules were retrospectively analyzed by TIC. Maximum intensity(IMAX), rise time(RT), time to peak(TTP) and the relative evaluated parameters of interior of the nodules, which were used the peripheral thyroid tissue as the reference, including relative IMAX(AI), relative RT(△RT) and TTP(ATTP) were achieved. Results The IMAX,AI of ma- lignant nodules (75.2±34.7,-17.7±34.9) were significantly less than in benign nodules (142.6±55.6,46.3±55.4)(P〈0.05). The △RT,△TTP of malignant nodules (〈 1 cm) (0.8±1.9,0.7±1.8)were significantly later than benign nodules (-0.3±1.7,-0.4±1.6) (P〈0.05), and the △RT (0.8 ± 1.9) was significantly later than malignant nodules(〉l cm) (-0.4±1.3) (P〈0. 05). Conclusions CEUS is of certain value to the differential diag- nosis of benign and malignant thyroid nodules.
