摘 要:目的 三维斑点追踪技术(3D-STE)评价缺铁性贫血(IDA)患者的左心室(LV)心肌收缩功能。方法 65例IDA患者按照血红蛋白(Hb)的浓度分为两组:B组Hb〉90g/L,C组Hb 60-90g/L。同时收集30例健康人作为对照组A组。对3组进行常规超声心动图检查,三维超声测量舒张末期左室容积(LVEDV)、收缩末期左室容积(LVESV)、左室射血分数(LVEF),并计算左室质量指数(LVMI),3D-STE分析左心室整体纵向应变(GLS)、整体面积应变(GAS)、整体径向应变(GRS)和整体周向应变(GCS)。结果 C组与B、A组间比较,LVEDV、LVESV和LVMI均升高,差异具有统计学意义(P均〈0.05);GRS、GCS、GLS和GAS均减低,差异有统计学意义(P均〈0.05)。结论 3D-STE能够有效地评估缺铁性贫血患者左心室心肌收缩功能的改变。Objective The aim of this study was to assess the impact of left ventricular (LV) systolic function in patients with iron deficiency anemia (IDA) by 3D speckle tracking echocardiography (3D-STE). Methods Sixty-five patients who were diagnosed as IDA were divided into 2 groups with hemoglobin levels. Group B: (Hb〉90 g/L), Group C: (Hb 60--90 g/L). We also enrolled 30 healthy control subjects as Group A. The parameters of the conven- tional echocardiography and 3D-STE were obtained respectively. Left ventricular end-diastolic volume(LVEDV), left ventricular end-systolic volume(LVESV), left ventricular ejection fraction (LVEF), left ventricular mass index(LV- MI)were measured and calculated by 3D technology. We used 3D-STE to analyze left ventricle global longitudinal strain (GLS), global area strain (GAS), global radial strain (GRS) and global circumferential strain (GCS). Results The LVMI, LVEDV and LVESV of group C were increased significantly compared with those of groups A and B(all P〈0.05). Compared with groups A and B, GCS, GRS, GLS and GAS of group C were significantly decreased (all P 〈0.05). Conclusions 3D-STE can effectively assess LV systolic function in patients with IDA.
