超声诊断的胎儿孤立性股骨短小的临床分析

摘　　要：目的探讨孕中期经超声诊断的孤立性股骨短小(ISF)胎儿的临床预后。方法回顾性分析于我院行产前超声检查诊断为ISF胎儿67例,并进行产后追踪随访,通过与正常对照组比较,评价ISF胎儿的预后情况。结果产前超声诊断并活产,产后资料完整的ISF胎儿67例,同时纳入对照组200例。ISF组患儿出生体质量低于对照组(2 787.2g vs 3 124.5g,P<0.01),其小于胎龄儿、低出生体质量儿发生概率(分别为17.9%vs 7.0%及19.4%vs 8.0%,P<0.05)、早产率(17.9%vs 9.0%,P<0.05)及新生儿重症监护室住院率(20.9%vs 7.5%,P<0.05)均高于对照组。结论孕中期发现的孤立性股骨短小胎儿出生后发生小于胎龄儿、低出生体质量儿的风险增高,提高对该疾病的认识为产前咨询及孕期管理具有一定的临床意义。Objective To estimate the prognosis of fetal isolated short femur （ISF） in the second-trimester ultra- sound examination. Methods Clinical data of fetuses with ISF diagnosed in our hospital were retrospectively analyzed. Postnatal follow-up was also conducted, and outcomes of interest were observed compared to that of fetuses with nor- mal femur length. Results Totally 67 ISF fetuses were born and successfully followed up, and 200 were controls. Fe- tuses with ISF had a lower birth weight（2 787.2 g versus 3 124.5 g, P〈0.01）. Even more, they were also associated with an increased risk for small for gestational age neonates （17.9% versus 7.0%, P〈 0.05）, low birth weight （19.4% versus 8. 0%, P〈0.05）, preterm birth（17.9%versus 9.0%,P〈0.05） and neonatal intensive care unit ad- missions （20.9% versus 7.5%, P〈0.05）. Conclusions Fetus with ISF in the second-trimester ultrasound examina- tion is associated with a subsequent delivery of small for gestational age and Low birth weight neonates. It has clinical significance for prenatal counseling and pregnancy management.
