摘 要:目的探讨玻璃酸钠联合氨基葡萄糖对膝骨性关节炎(KOA)关节镜术后膝关节功能的影响。方法选取2012年10月至2013年11月行膝骨性关节炎关节镜术的患者88例,按治疗方式的不同分为3组。玻璃酸钠组(26例)患者单纯使用玻璃酸钠治疗,氨基葡萄糖组(26例)患者单纯使用氨基葡萄糖治疗,联合治疗组(36例)患者联用玻璃酸钠与氨基葡萄糖治疗。比较治疗60 d后3组患者的临床疗效、膝关节评分、疼痛程度及不良反应发生情况。结果治疗60 d后,联合治疗组总有效率为86.11%,显著高于玻璃酸钠组的53.85%及氨基葡萄糖组的57.69%(P<0.05);各组患者Lysholm膝关节功能评分均较治疗前升高(P<0.01),且联合治疗组显著高于玻璃酸钠组及氨基葡萄糖组(P<0.05);各组疼痛视觉模拟评分法(VAS)评分均较治疗前降低(P<0.01),且氨基葡萄糖组与联合治疗组均低于玻璃酸钠组(P<0.05);治疗过程中,患者均未出现严重不良反应。结论玻璃酸钠联合氨基葡萄糖治疗KOA安全、有效,可有效恢复患者膝关节功能,且安全性高,值得临床推广。Objective To investigate the efficacy of sodium hyaluronate combined with amino glucose on knee joint function after arthroscopic surgery in patients with knee osteoartbritis. Methods 88 cases of patients from October 2012 to November 2013 with knee osteoarthritis and arthroscopic surgery in the hospital were selected and divided into 3 groups according to different treatment methods. There were sodium hyaluronate group (26 cases):sodium hyaluronate treatment;glucosamine group (26 cases):the simple use of glucosamine therapy;combined treatment group (36 cases):the use of sodium hyaluronate combined with glucosamine treatment. The clinical efficacy, knee score, pain degree and adverse reaction of the three groups were compared after 60 d of treatment. Results 60 d after treatment, the total effective rate in the combined treatment group was 86.11%, which was significantly higher than the sodium hyaluronate group (53.85%) and glucosamine group (57.69%) (P 〈 0.05).The Lysholm knee function scores of all groups were significantly higher than that before treatment (P 〈 0. 01),and the score of combined treatment group was significantly higher than that of sodium hyaluronate group and glucosamine group (P 〈 0.05 ). The VAS scores of each group after treatment was significantly lower than that before treatment (P 〈 0. 01),and the combined group was significantly higher than the sodium hyaluronate group and glucosamine group (P 〈 0. 05 ) ; After treatment,the pain score of VAS of Amino glucose group and combined therapy group was lower than that of sodium hyaluronate group. There were no serious adverse reactions in 88 patients during the course of treatment. Conclusion Sodium hyaluronate combined with amino glucose is safe and effective treatment for KOA; it has good curative effect, and can effectively restore the function of knee joint,which is worthy of clinical promotion.
