浓替硝唑含漱液治疗下颌智齿冠周炎临床研究

摘　　要：目的观察并评价浓替硝唑含漱液治疗下颌智齿冠周炎的临床疗效。方法选择2014年5月至2016年3月收治的急性局限型下颌智齿冠周炎患者114例,随机分为对照组和观察组,各57例。对照组采用浓替硝唑含漱液治疗,对照组采用丁硼乳膏治疗,观察两组患者临床疗效、细菌学指标变化及不良反应的发生情况。结果观察组总有效率为98.25%,显著高于对照组的84.21%(P<0.05);观察组不良反应发生率为7.02%,显著低于对照组的15.79%(P<0.05)。治疗后,观察组千倍视野下细菌学密度为(49.74±10.36)个,显著低于对照组的(67.48±9.29)个(P<0.05);观察组螺旋体占比为(6.85±1.14)%,显著低于对照组的(16.47±2.69)%(P<0.05);观察组革兰阳性菌占比为(61.45±5.48)%,显著高于对照组的(46.58±5.47)%(P<0.05);观察组革兰阴性菌占比为(30.95±5.58)%,显著低于对照组的(38.45±6.14)%(P<0.05)。结论急性局限型智齿冠周炎患者采用浓替硝唑含漱液治疗效果较好,细菌清除效果良好,且不良反应程度较轻,值得临床推广。Objective To investigate the clinical efficacy of Concentrated Tinidazole Gargles treatment on pericoronitis. Methods 114 cases of acute wisdom tooth pericoronitis patients from May 2014 to March 2016 were randomly divided into the control group and the observation group,57 cases in each group. The observation group used Concentrated Tinidazole Gargles treatment,the control group uses Dingpeng cream treatment. The clinical efficacy, bacteriological index changes and adverse reactions in the two groups were observed. Results The total effective rate of the observation group （98.25%） was significantly higher than that of the control group （84. 21%, P 〈 0. 05）,the incidence of adverse reactions （7.02%） was significantly lower than that of the control group （15.79%, P 〈 0. 05）,the difference between the two groups was statistically significant （P 〈 0.05）. After treatment,the bacterial density under 1 000 x microscope of the observation group was （49.74 ± 10.36）, which was significantly lower than （67.48 ± 9.29） of the control group;the proportion of spirochete in the observation group was （6. 85±1.14）% ,which was significantly lower than （16.47 ±2.69）% of the control group （P 〈 0.05）;the proportion of gram positive bacteria accounted for （61.45 ± 5.48）% in the observation group, which was significantly higher than（46.58±5.47）% detected in the control group （P 〈 0.05）;the proportion of gram negative bacteria accounted for （30. 95 ±5.58）% in the observation group,which was significantly lower than （38.45±6.14）% in the control group （P 〈 0.05）. Conclusion Concentrated Tinidazole Gargles for treating patients with acute wisdom tooth pericoronitis has good effect,can clear the bacteria with less adverse reactions,which is worthy of clinical promotion.
