手术切除联合自体角膜缘干细胞移植降低角膜缘翼状胬肉复发效果评价

摘　　要：目的探讨手术切除联合自体角膜缘干细胞移植在降低角膜缘翼状胬肉复发率中的应用价值。方法选取医院2014年7月至2016年2月收治的翼状胬肉患者110例122眼,根据治疗方式的不同分为观察组(52例59眼)和对照组(58例63眼),观察组给予翼状胬肉切除术联合自体角膜缘干细胞移植,对照组给予翼状胬肉切除术联合生物羊膜移植,比较两组患者的临床疗效和复发率。结果术后7 d、1个月、3个月,两组患者的视力和屈光度均较术前有明显改善,差异有统计学意义(P<0.05),但组间同时点比较,差异无统计学意义(P>0.05);观察组复发率为3.39%,明显低于对照组的11.11%,差异有统计学意义(P<0.05)。结论手术切除联合自体角膜缘干细胞移植治疗角膜缘翼状胬肉临床疗效肯定,可有效降低术后复发率,值得临床推广。Objective To explore the application value of surgical excision combined with autologous corneal limbal stem cell transplantation in reducing the recurrence rate of pterygium. Methods 110 cases （122 eyes） of patients with pterygium treated in our hospital from July 2014 to February 2016 were selected and divided into the observation group （52 case,59 eyes） and the control group （58 cases,63 eyes） according to treatment methods. The observation group was given surgical excision combined with autologous corneal limbal stem cell transplantation,while the control group was treated by surgical excision combined with biological amniotie membrane transplantation. The clinical efficacy and the recurrence rate of pterygium in the two groups were compared. Results For both groups, the vision and diopter of postoperative 7 d, 1 and 3 months were improved significantly compared with that of preoperative, the difference was of statistical significance （P 〈 0. 05）;there was no significant difference between the two groups in the vision and diopter of postoperative 7 d, 1 and 3 months （P 〉 0. 05）;the recurrence rate of pterygium in the observation group was 3.39% ,which was significant lower than 11.11% in the control group （P 〈 0. 05）. Conclusion Surgical excision combined with autologous corneal limbal stem cell transplantation in reducing the recurrence rate of pterygium has good efficacy, can effectively reduce the recurrence rate,which is worthy of clinical promotion.
