可吸收棒治疗跖骨头骨折伴跖趾关节脱位临床疗效分析

摘　　要：目的探讨跖骨头骨折伴跖趾关节脱位切开复位可吸收棒内固定治疗的临床疗效。方法回顾性分析2011年5月-2015年1月我科治疗的32例跖骨头骨折伴跖趾关节脱位病人的临床资料，其中男27例，女5例，年龄15～76岁（平均40．16岁）。均采用足背纵形微切口，术中根据骨折情况采用可吸收棒行内固定治疗，分别于术后第1、3、6、9、12个月定期门诊复查并摄x片，采用美国足踝骨科协会（AOFAs）足踝评分系统评价手术疗效。结果切口愈合率100％，末次随访时按美国足踝骨科协会足踝评分标准，术前平均得分为（47．16±6．45），术后平均得分为（93．61±7．18），与术前评分具有显著性差异（P〈0．05）；其中优26例，良6例，优良100％，无相关并发症。结论可吸收棒在跖骨头骨折伴跖趾关节脱位的治疗中具有显著优势，其具有良好生物相容性，操作简便，勿需二次手术取出，配合术后早期功能锻炼，疗效颇为满意，值得临床推广应用。Objective To explore the clinical effect of application of absorbable rods for the treatment of fractures of head of metatarsal associated with dislocation ofmetatarsophalangealjoint. Methods Retrospective analysis of 32 cases in our department from May 2011 to Jan 2015, including 27 men and 5 women, ages 15 to 76 years old （average age 40.16）. Using dorsal longitudinal micro incision, and according to the situation of fracture to use absorbable rods inline fixed treatment during the operation. The patients gradually strengthen the limb function exercise and be taken X-ray after surgery in the clinic at 1, 3, 6, 9 and 12 months. Using AOFAS evaluation criteria for analyzing the recovery condition. Results The incision healing rate was 100 %, in accordance with AOFAS evaluation criteria for scoring at the last follow- up, the excellent and good rate comes to 100%. Conclusion Absorbable rods in treatment of the fractures of head of metatarsal associated with dislocation of metatarsophalangeal joint has a significant advantage, it has good biological compatibility and easy to operate. At the same time, it does not need to secondary surgery to remove, and it is good for patients＇ early postoperative functional exercise. In conclusion, it is satisfied of the clinical curative effect and is worthy of popularization and application.
