中西医结合治疗2型糖尿病气阴两虚证临床观察

摘　　要：目的:观察津力达颗粒对2型糖尿病气阴两虚证患者的临床症状、血糖、胰岛功能及胰岛素抵抗的影响。方法:58例2型糖尿病患者,在原有治疗方案基础上,强化饮食运动教育,同时加用津力达颗粒,每次9 g,3次·d-1,疗程2个月,进行治疗前后对照。结果:治疗后患者临床症状积分较治疗前明显减少(P<0.001),说明患者临床症状明显改善;空腹血糖较治疗前下降2.74 mmol·L^(-1)(P<0.001)、餐后2小时血糖下降10.13 mmol·L^(-1)(P<0.001)、糖化血红蛋白下降1.67%(P<0.001);餐后2小时胰岛素水平升高7.15 m U·L^(-1)(P<0.001);胰岛素分泌指数升高较治疗前0.13(P<0.001),胰岛素抵抗指数则下降0.81(P<0.05);而空腹胰岛素水平及患者体质量指数未见明显变化。结论:津力达颗粒可改善2型糖尿病气阴两虚证患者临床症状,能够一定程度恢复患者胰岛β细胞功能、促进餐后胰岛素分泌,可改善胰岛素抵抗,具有较好的降糖作用。Objective： To observe the clinical effect of Jilida Particle on the clinical symptoms,blood glucose,islet function and insulin resistance in treatment of Type 2 Diabetes with Qi and Yin Deficiency Syndrome. Method： 58 cases of Type 2 diabetic patients were chosen and were treated with the Integrative medicine. Specially,based on the original treatment,the diet and exercise education was strengthened and Jinlida particle was used in treatment for 2 months. Clinical results were compared before and after treatment. Results： After treatment,the clinical symptoms score after treatment was significantly reduced（ P〈0. 001）,which suggests that the clinical symptoms improved significantly. The fasting plasma glucose decreased 2. 74 mmol·L^-1（ P〈0. 001） compared with that before treatment; the 2-hour postprandial blood glucose decreased 10. 13 mmol·L^-1（ P〈0. 001） compared with that before treatment; the hemoglobin decreased by 1. 67%（ P〈0. 001） compared with that before treatment; the 2-hour postprandial insulin levels rose 7. 15 m U·L^-1（ P〈0. 001） compared with that before treatment; the insulin secretion index rose by 0. 1（ P〈0. 001） compared with that before treatment; the insulin resistance index fell by 0. 81（ P〈0. 05） compared with that before treatment. There were no significant changes in fasting insulin level and body mass index of patients. Conclusion： Jinlida particle can significantly improve patients＇ clinical symptoms of Type 2 Diabetes with Qi and Yin Deficiency Syndrome. It also can restore the islet β-cell function,promote the postprandial insulin secretion,improve the insulin resistance condition to some extents,and exert a significant hypoglycemic effect in treatment of Type 2 Diabetes.
