蒲鹿清消汤加四黄散外敷治疗早期乳腺炎临床研究

摘　　要：目的：观察蒲鹿清消汤加外敷四黄散治疗早期乳腺炎的临床疗效。方法：110例早期乳腺炎患者随机分为观察组和对照组各55例。对照组予以五水头孢唑林钠1.0 g,2次·d^-1,静脉滴注。观察组在对照组基础上加用蒲鹿清消汤和四黄散外敷,疗程均为6 d。比较两组患者中医证候变化、临床乳腺炎相关检验指标以及总体疗效评价。结果：治疗后观察组乳房胀痛、局部皮肤灼热、乳房结块、恶寒发热、烦躁口渴、脉数和身痛头痛积分显著低于对照组（P〈0.05）。两组治疗后白细胞、中性粒细胞百分比、C反应蛋白（C-reactive protein,CRP）和血沉（erythrocyte sedimentation rate,ESR）均较治疗前降低（P〈0.05）,观察组治疗后降低更显著（P〈0.05）。观察组有效率为100.00%,对照组有效率为83.64%,两组比较,差异有统计学意义（χ^2=8.985 1,P=0.002 7〈0.05）;观察组继续哺乳率98.18%,对照组80.00%,两组比较,差异有统计学意义（χ^2=8.574 3,P=0.003 4〈0.05）。结论：蒲鹿清消汤加四黄散外敷治疗早期乳腺炎可快速改善临床症状,降低炎症指标,临床疗效可靠。Objective： To observe the clinical curative effect of Pu Lu Qing Xiao Tang and Sihuang San external application treatment of early mastitis. Methods： 110 cases of early mastitis were randomly divided into observation group and control group with 55 cases in each group. The control group received Cefazolin Sodium Pentahydrate 1. 0g,2 times a day,intravenous drip. The observation group based on the control group additionally used Pu Lu Qing Xiao Tang and Sihuang San external application,and the course of treatment was 6 days. Compared TCM symptomatic changes,clinical mastitis related indicators and overall efficacy evaluation of the two groups of patients. Results： After treatment,breast pain,breast lumps,skin burning,chillness and fever,agitated and thirsty,pulse and body pain,headache score of the observation group was significantly lower than the control group（ P〈0. 05）. After treatment,white blood cells,the percentage of neutrophils,C reactive protein（ CRP） and erythrocyte sedimentation rate（ ESR） were lower than those before treatment（ P〈0. 05）,the patients in the observation group decreased significantly（ P〈0. 05）. The effective rate of observation group was 100. 00%,and the control group was 83. 64%,and the difference was statistically significant（ χ^2= 8. 985 1,P = 0. 002 7 0. 05）; continued breast-feeding rate of the observation group was 98. 18%,80. 00% in the control group,and the difference was statistically significant（ χ^2= 8. 574 3,P = 0. 003 4 0. 05）. Conclusion： Pu Lu Qing Xiao Tang and Sihuang San external application treatment of early mastitis can rapidly improve clinical symptoms,reduce inflammation index,and clinical curative effect is reliable.
