腰椎间盘表观弥散系数与肿瘤坏死因子-α、白细胞介素-6的相关性研究

摘　　要：目的探讨腰椎间盘突出症患者致痛椎间盘的表观弥散系数值(ADC)与肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)的相关性。方法 25例因腰椎间盘突出症手术患者(男15例,女10例)的30个髓核组织作为研究对象。术前腰椎MRI常规序列评价其退变程度(Pfirrmann分级),弥散加权成像测量其平均ADC值,采用酶联免疫吸附法(ELISA)测定其TNF-α和IL-6蛋白含量。结果 30例手术腰椎间盘中,Ⅲ级10例,Ⅳ级20例,致痛椎间盘髓核区可见TNF-α、IL-6表达。同一退变级别(Ⅲ、Ⅳ)L5/S1椎间盘ADC值、TNF-α和IL-6蛋白含量均高于L4/5,进一步t检验结果显示:除了Ⅳ级,L4/5与L5/S1椎间盘ADC值、TNF-α含量差异有统计学意义(tIv/ADC=-2.52,tIV/TNF-α=-2.21,均P<0.05)外,其他差异均无统计学意义。结论致痛椎间盘平均ADC值与其TNF-ɑ、IL-6表达相关。Objective To assess the correlationbetween the apparent diffusion coefficient （ADC） values of lumbar intervertebral disc and tumor necrosis factor-α （TNF-α） and interleukin-6 （IL-6） in patients with lumbar disc herniation. Methods 30 nucleus pulposues from 25 patients with lumbar disc herniation （ 15 men and 10 women） treated with surgery were included in this study. Conventional MRI sequence was implemented to evaluatethedisc degeneration. Diffusion weighted imaging （DWI） was performed to estimate the mean ADC value of lumbar intervertebral disc. The content of TNF-α and IL-6 in nucleus pulposues were measuredwith enzyme-linked immunosorbent assay （ELISA）. Results There were 10 lumbar disc herniations on III grade and 20 lumbar disc herniations on IV grade in 30 lumbar disc herniations.Nucleus pulposues with lumbar disc herniation treated with surgery expressed TNF-α and IL-6 proteins. Mean ADC values, the content of TNF-α and IL-6 proteins in L5/S1 discs treated with surgery in the same grade （III and IV） were higher in L4/5 discs. However, except for the significant differences in the ADC values and the content of TNF-α proteins between L4/5 and LS/S1 discs （tIV/ADC = -2.52, tIV/TNF-α = -2.21, P 〈 0.05）, no significant differences were observed between other groups. Conclusion The mean ADC values of lumbar intervertebral disc in patients with lumbar disc herniation are correlatedwith the content of TNF-α and IL-6 proteins.
