甲泼尼龙联合丙种球蛋白佐治狼疮性肺炎1例报告并文献复习

摘　　要：目的探讨甲泼尼龙联合丙种球蛋白佐治狼疮性肺炎的疗效。方法对1例系统性红斑狼疮伴狼疮性肾炎并发狼疮性肺炎患者,在基础治疗的基础上,静脉给予甲泼尼龙联合丙种球蛋白。结果治疗后,患者气促症状较前改善,但仍需呼吸机通气,肺间质性改变,限制肺部通气换气功能,血红蛋白下降严重,治疗过程中出现了一些不良反应,如消化道出血、钠水潴留、白细胞反应性升高。结论甲泼尼龙在治疗狼疮性肺炎时可以在一定程度上控制病情延长生命,但也有一定的不良反应,临床应用时应严密观察。Objective To investigate the curative effect of methylprednisolone combined with gamma globulin on lupus pneumonia.Methods A patient with systemic lupus erythematosus,lu-pus nephritis and lupus pneumonia were intravenously given methylprednisolone and gamma glob-ulin based on the basic treatment.Results After treatment,this patient achieved an improvement in anhelation,but still required mechanical ventilation.Furthermore,pulmonary interstitial change,restrictive pulmonary ventilati and severe decrease in hemoglobin level were found in this patient.Moreover,some adverse reactions occurred during treatment,such as gastrointestinal bleeding,sodium and water retention,and reactive leukocyte increase.Conclusion Treatment with methylprednisolone can control disease and prolong life in patients with lupus pneumonia. However,it is also associated with some adverse reactions,which must be observed intensively.
