摘 要:目的:通过评价慢性阻塞性肺疾病(COPD)合并继发性红细胞增多症患者肺功能,观察骨髓单个核细胞凋亡及Bcl-2 mRNA的表达情况,探讨其在COPD发病中的作用。方法:选取COPD和COPD合并继发红细胞增多症患者共47例,分别为COPD组(n=32)和COPD+继发红细胞增多症组(n=21),30例单纯骨折患者作为对照组,肺功能仪测定FEV1/FVC(%)和肺一氧化碳弥散量(DLco),流式细胞仪测定骨髓单个核细胞凋亡率,RTPCR方法检测Bcl-2 mRNA的表达。结果:COPD+继发红细胞增多症组FEV1/FVC(%)和DLco均低于COPD组,mMRC呼吸困难分级评分高于COPD组,骨髓单个核细胞凋亡率低于对照组和COPD组,Bcl-2 mRNA的相对表达高于对照组和COPD组,上述差异均有统计学意义(P<0.05)。相关性分析提示COPD+继发红细胞增多症组中HB蛋白浓度与骨髓单个核细胞凋亡率和Bcl-2 mRNA的表达呈负相关(P<0.05)。结论:COPD合并继发红细胞增多症可降低患者的肺功能,增加呼吸困难的程度,Bcl-2家族分子可能参与了COPD合并继发红细胞增多症患者骨髓单个核细胞凋亡异常的机制。Objective To evaluate pulmonary function and observe bone marrow mononuclear cells (BM- MNCs) apoptosis and Bcl-2 mRNA expression in chronic obstructive pulmonary disease (COPD) with secondary polycythemia (SP) patients. Methods There are 30 simple fracture patients as the control group to compare 32 COPD patients (COPD group) and 21 COPD with SP patients (COPD + SP group). Respiratory function (FEV1/FVC and DLco) of patients was evaluated using the spirometer. The apoptosis rates of BM-MNCs in all groups were measured by flow cytometry. Expression levels of Bcl-2 mRNA of BM-MNCs with all groups were detected by RT-PCR. Results Respiratory function (FEV1/FVC and DLco) of COPD + SP group were lowerthan COPD group and mMRC score were greater than COPD group (P 〈 0.05). Apoptosis rate of BM-MNCs with COPD + SP were significant lower than the control group and COPD group and Expression levels of Bcl-2 mRNA BM-MNCs were significant higher than the control group and COPD group (P 〈 0.05). Conclusions Our results suggest that SP affect on respiratory function (FEV1/FVC and DLco) and increase the degree of dyspnea of COPD patients. Bcl-2 family molecules may be involved in BM-MNCs abnormal apoptosis mechanism in COPD with SP patients.
