摘  要:目的:探讨一氧化氮合酶(NOS)、神经肽Y(NPY)以及脐带组织细胞凋亡与妊娠期肝内胆汁淤积症(ICP)发病的相关研究。方法:随机选取33例重度ICP患者为研究组,35例健康孕妇为对照组,分别检测两组脐带组织和血浆中NOS、NPY的表达情况,并检测脐带组织细胞的凋亡指数(Al)。结果:NOS在ICP组脐带和脐血浆的表达均明显低于对照组,NPY在ICP组脐带和脐血浆的表达均明显高于对照组,差异均具有统计学意义(P<0.001);ICP组脐带细胞的AI明显高于对照组,ICP组脐带组织线粒体的荧光强度表达明显高于对照组,差异均具有统计学意义(P<0.001)。结论:ICP组NOS、NPY的异常表达和细胞凋亡可能与ICP的发病及胎死宫内有关,监测这些指标对病情判断及指导治疗具有重要意义。Objective To investigate the role of NOS, NPY and apoptosis in the development of ICP. Methods NOS and NPY were detected by immunohistochemistry in umbilical tissue and ELISA method in umbilical blood plasma from 68 cases selected randomly (33 cases of severe ICP and 35 normal pregnant women). Apoptosis was detected by Tunnel in umbilical tissues. Results The expression of NOS in study group was significantly lower than the control group in the umbilical tissue and the umbilical vein plasm, the expression of NPY in study group was significantly higher than the control group in the umbilical tissue and the umbilical vein plasm, the difference was statistically significant (P 〈 0.001); AI in study group was significantly higher than the control group in the umbilical tissue, the difference was statistically significant (P 〈 0.001 ). The fluorescence intensity of the umbilical cord tissue in the study group was significantly higher than the control group in the umbilical tissue, the difference was statistically significant (P 〈 0.001 ). Conclusions The abnormal expression of NOS , NPY and apoptosis may be associated with the development of ICP and intrauterine death, it is important to monitor the changes for the judgment of the disease and treatment.
