摘　　要：目的:探讨腰腿痛患者腰椎间盘重度退变(SLDD)的危险因素。方法:回顾性分析340例腰腿痛患者,根据Pfirrmann法将每一节段腰椎间盘退变(LDD)程度分为5级,并将患者分为非SLDD组(n=125)及SLDD组(n=215),进行统计学分析。结果:SLDD组与非SLDD组相比,BMI、高血压患病率、糖尿病患病率更高(P<0.05),年龄、吸烟、酗酒、职业无统计学差异(P>0.05)。校正可能混杂因素后,年龄与各椎间盘退变程度均正相关(P<0.05),BMI与L3/4、L4/5、L5/S1退变程度正相关(P<0.05);多元逻辑回归分析:年龄是各节段SLDD的危险因素(P<0.05),高BMI是L3/4、L4/5、L5/S1 SLDD的危险因素(P<0.05)。结论:年龄为SLDD的危险因素,BMI与LDD程度相关,高BMI是下腰椎SLDD的危险因素。Objective To investigate the risk factors of severe lumbar disc degeneration （SLDD） in low- back pain or leg pain patients. Methods MRI data of 340 patients were retrospectively analyzed. Lumbar disc degeneration （LDD） was classified into five grades by MRI according to the method proposed by Pfirrmann. Patients were divided into Non-SLDD group （n = 125） and SLDD group （n = 215）. The correlation between factors and LDD and the risk factors of SLDD were statistically analyzed. Results Compared with those in the Non-SLDD group, BMI, morbidity rate of hypertension and morbidity rate of diabetes were significant higher in SLDD group （P 〈 0.05）. There was no significant difference of age, smoking, alcoholic addiction or occupation between the two groups （P 〉 0.05）. After adjusting for potential interactions, age was positive correlated with every lumbar disc （P 〈 0.05） and BMI was positive correlated with degeneration degree of L3/4, IA/5 and LS/S1 （P 〈 0.05）. Multivariate logistic regression analysis showed that age was a risk factor of SLDD in every lumbar disc （P 〈 0.05） and BMI was a risk factor of SLDD at L3/4, L4/5 and L5/S1 （P 〈 0.05）. Conclusion Age is a risk factor of SLDD. BMI is correlated with LDD. Higher BMI is a risk factor of SLDD at L3/4, IA/5, L5/S1.
