胱抑素C与早期动脉粥样硬化的相关性研究

摘　　要：目的观察早期动脉粥样硬化患者血清胱抑素C(Cys C)水平变化及其对早期动脉粥样硬化诊断的临床意义。方法选取2015年9月—2016年1月皖南医学院附属弋矶山医院体检者112例,通过测量受检者臂踝脉搏波传导速度(ba-PWV)评估其早期动脉硬化程度,根据左右两侧baPWV值分为对照组和动脉硬化组。利用全自动生化分析仪检测受检者外周血Cys C、尿酸(UA)和血脂等水平。结果 (1)动脉硬化组血清胱抑素C水平较对照组升高[左:(1.05±0.18)mg/L vs.(0.97±0.16)mg/L,P<0.05;右:(1.04±0.17)mg/L vs.(0.97±0.17)mg/L,P<0.05];(左侧)动脉硬化组与对照组比较:2组年龄、BMI、TC、LDL-C比较差异有统计学意义(P<0.05),而TG、HDL-C、BS及UA比较差异无统计学意义(P>0.05);与对照组比较,(右侧)动脉硬化组患者年龄、体质量指数(BMI)及总胆固醇(TC)、三酰甘油(TG)差异有统计学意义(t=-4.497、-2.781、-2.644、-2.319,P<0.05),而血糖(BS)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL,C)及尿酸(UA)比较差异无统计学意义(P>0.05);(2)Cys C与动脉硬化指标中年龄、TC、TG呈正相关,且与年龄相关性最大(r=0.358、0.225、0.257,P<0.01)。结论动脉硬化组Cys C水平高于对照组,且与年龄及总胆固醇、三酰甘油呈正相关。Cys C水平对早期动脉粥样硬化患者的前期预防性诊断具有一定临床意义。Objective To observe the changes of serum cystatin C（Cys C）level in patients with early atherosclerosis,and investigate the potential clinical significance of Cys C level on diagnosis in patients with early atherosclerosis.Methods One hundred and twelve cases from September 2015 to January 2016 in Wangnan Medical College Affiliated Yijishan Hospital were enrolled.Brachial ankle pulse wave velocity（ba-PWV）of the subjects were measured to evaluate the extent of atherosclerosis in patients with early stage.Automatic biochemical analyzer was used to detect the levels of the subject＇s peripheral blood cystatin C（Cys C）,uric acid（uric acid,UA）,serum lipid and so on.Results（1） The cystatin C level in atherosclerosis group was higher than that of non-atherosclerosis group[left;（1.05 ±0.18）mg/L vs.（0.97 ±0.16） mg/L,P〈0.05;right：（1.04±0.17）mg/Lvs.（0.97±0.17）mg/L,P〈0.05].The age,BMI and total cholesterol（TC）,LDL-Cin atherosclerosis group compared with that of control group,the difference was statistically significant（P〈0.05）.However,the TG.high density lipoprotein（HDL-C）,BS and uric acid（UA） level in atherosclerosis group compared with that of non-atherosclerosis group,there is no significant difference（P〈0.05）.Compared with the control group,the age,BMI,TC,TG of the two groups were statistically different（t =-4.497,-2.781,-2.644,-2.319,P〈0.05）,but there was no significant difference between BS,HDL-C,LDL-C and UA（P〉0.05）.（2） There was a positive correlation between Cys C and age,TC and TG in arteriosclerosis index,and there was no significant correlation with age（r =0.358,0.225,0.257,P〈0.01）.Conclusion The level of Cys C in the atherosclerosis group was higher than that in the control group,and was positively correlated with the total cholesterol and the age of three.Cys C level has a certain clinical significance for early diagnosis of atherosclerosis in patients with early atherosclerosis.
