摘  要:目的探讨采用动力髋螺钉(DHS)、空心钉结合斜T形钢板单皮质固定治疗股骨粗隆间粉碎性骨折的疗效。方法笔者自2013-01-2015-10采用DHS、空心钉结合斜T形钢板单皮质固定治疗股骨粗隆间粉碎性骨折18例,观察手术时间、术中出血量及术后并发症发生情况。结果手术时间60~120min,平均80min;术中出血量50-200ml,平均80ml。18例均获随访6~36个月,平均12个月。骨折愈合良好,无股骨颈短缩、内固定松动、髋内翻、股骨头坏死等并发症。术后6个月疗效采用髋关节功能Harris评分评定均为优。结论采用DHS、空心钉结合斜T形钢板单皮质固定治疗股骨粗隆间粉碎性骨折。具有固定牢靠、骨折易愈合、内固定松动并发症少等优点,疗效满意。Objective To investigate the effect of dynamic hip screw (DHS) and hollow screw combined with single cortical fixation by oblique T-shaped plate for the treatment of intertrochanteric comminnted fractures. Methods From January 2013 to October 2015, 18 eases of intertrochanteric comminuted fractures were treated with DHS and hollow nail combined with single cortical fixation by oblique T-shaped plate. The operative time, intraoperative blood loss and postoperative complications were observed. Results The operation time was 60-120 min, with an average of 80 min. Intraoperative blood loss was 50-200 ml, with an average of 80 ml. All 18 cases were followed up for 6 to 36 months, with an average of 12 months. Fractures healed well, without femoral neck shortening, internal fixation loosening, hip inversion, and femoral head necrosis. After 6 months. The efficacy of hip joint function Harris score was excellent. Conclusion The DHS and hollow screw combined with single cortical fixation by oblique T-shaped plate for the treatment of femoral intertrochanteric comminuted fracture, can achieve a satisfactory effect with the advantages of firm fixation, easier fracture healing, and less loosening of fixation.
