乙肝五项与乙肝HBV-DNA荧光定量检测及肝脏纤维化指标的相关性分析

摘　　要：目的:分析乙肝HBV-DNA荧光定量检测及肝脏纤维化指标与乙肝五项结果的一致性,探讨三者的相关性。方法:分别采用ELISA、PCR、放射免疫法对46例乙肝患者进行乙肝五项、HBV-DNA、肝脏纤维化指标的检测。结果:在HBsAg(+)HBeAg(+)HBcAb(+)的模式下,HBV-DNA的阳性率为90%,在HBsAg(+)HBeAb(+)HBcAb(+)的模式下,HBV-DNA的阳性率为60%。在HBsAg(+)HBeAg(+)HBcAb(+)和HBsAg(+)HBeAb(+)HBcAb(+)两种模式下,HBV-DNA阳性率、肝脏纤维化指标、肝功能、血小板等指标差异均有统计学意义(P<0.05)。结论:乙肝HBV-DNA含量、肝脏纤维化指标与乙肝五项的结果密切关联,联合检测乙肝HBV-DNA荧光定量检测及肝脏纤维化指标能很好的反映乙肝病毒的复制及传染性,肝纤维化程度及发生肝硬化的趋势,对于乙肝的诊治具有指导意义。Objective：To study the concordance of detection among HBV-DNA by quantitative fluorescent detection, liver fibrosis indexes and HBV five serological markers, and to probe into the correlation between them. Methods： HBV five serological markers were detected by ELISA, HBV-DNA were detected by PCR and liver fibrosis indexes were detected by radioimmunoassay in 46 patients with hepatitis B virus. Results： The positive rates of HBV-DNA with HB- sag（ ＋ ） HBeAg （ ＋ ） HBcAb （ ＋ ） were 90 %. The positive rates of HBV-DNA with HBsAg （ ＋ ） HBeAb （ ＋ ） HBcAb （＋） were 600//00. Statistical significance in The positive rates of HBV-DNA, liver fibrosis indexes, liver function and platelet were found in the detection rates between the two groups（P^0. 05）. Conclusion： There is a close relationship between HBV-DNA by quantitative fluorescent detection and liver fibrosis indexes as well as HBV five serological markers. Combined detection of HBV-DNA by quantitative fluorescent detection and liver fibrosis indexes has a diag- nostic significance to reflect replication, infectivity, degree of hepatic fibrosis as well as the trend of liver cirrhosis. This study has guiding significance for the diagnosis and treatment of hepatitis B.
