摘　　要：目的了解重庆市医疗机构医务人员清洗消毒后的手卫生状况,比较不同科室、机构间存在的差异。方法采用分层抽样的方法,于2012-2014年对重庆市28家医疗机构医务人员手进行清洗消毒后采样,并就菌落数和合格率进行统计学分析。结果三年共计采样935人次,整体合格率为99.57%。不同科室、环境、医院间菌落数差异均有统计学意义(P<0.01),其中外科手术室和Ⅰ类环境菌落中位数均为0(0,0)cfu/cm2低于其他分组;私营医院菌落中位数为1(0,5)cfu/cm2高于其他类型医院,差异均有统计学意义(P<0.01)。结论 2012-2014年监测的28家医疗机构医务人员消毒后手卫生效果较好,但仍应注意部分医院的治疗室、口腔科及私营医院的医务人员手卫生质量,在洗手方法和院感控制上加强管理。Objective To invesigate the quality of hand hygiene after cleaning and disinfection among medical professionals of medical institutions in Chongqing City, and to compare the diversities among different departments and institutions. Methods A stratified sampling method was used to collect the samples from cleaning and disinfection hands of medical workers from 28 medical institutions in Chongqing City during 2012-2014. The colony count and the qualified rate were statistically analyzed. Results We sampled 935 person-time during the 3-year period, with the overall qualified rate of 99.57%. There were statistically significant differences in the colony count among different departments, environment and hospitals （ P〈0.01 ） , of which the median colonies of operation rooms and the environment I （ both 0（0,0） cfu/cm^2 ） were both lower than those of other groups. The median colony of private hospitals （ 1 （ 0,5 ） cfu/cm^2 ） was higher than those of other types of hospitals, with statistically significant differenees （ P〈0.01 ）. Conclusions The effects of hand hygiene after cleaning and disinfection among medical staff of 28 medical institutions in 2012-2014 are good, but more attention should be paid to treatment rooms and stomatology departments of some hospitals and the quality of hand hygiene among medical staff of private hospitals and to enhancing the management of hand-washing methods and nosocorsial infection control.
